Job Overview:

The Supervisor II reviews staff case documentation, and provides monthly supervision to each clinical staff member regarding caseload, crisis assessments, interventions and treatment plans.

Crisis Assessment Response Enhanced Services (CARES) is a statewide clinical outreach service for individuals with a dual diagnosis of mental illness/developmental disability (MIDD).

The CARES clinical team provides primary crisis response to screening centers. Referrals from other agencies and individuals are triaged for response. There are extensive follow-up interventions and crisis stabilization in the community. The primary goals are to manage behavioral disturbances, stabilize the crises and avert psychiatric emergency services. CARES team works alongside providers and families/caregivers to help develop behavior strategies and provide ongoing coaching, consultation and training.

Qualifications:

Required:

  • Master's degree in Counseling, Psychology, or related field

  • Clean drivers license as determined by employer and validated by NJMVA

  • At least one year of clinical work experience

Preferred:

  • LPC

Essential Functions:

  • Provide daily operations supervision to clinical staff regarding crisis referral assessments, interventions and treatment plans

  • Review intakes and ensures that staff responds by phone and face to face within the time frames designated in the state contract

  • Assigns cases to staff

  • Conduct meetings with clinical staff at least weekly to review cases as a team and to discuss strategies for treatment and intervention

  • Review discharges and ensure that there is justification for closing the case

  • Makes certain that there is linkage to appropriate services

  • Review staff documentation, including assessments, treatment plans and case recording

  • Recommends revisions in the case recording for clinical appropriateness and other documentation

  • Makes certain that the charts are in order and the necessary forms are completed

  • Intervene with DDD case managers and supervisors as needed regarding problem situations

  • Attends IDT meetings when necessary for administrative issues including placement problems

  • Assist program director in implementing and monitoring client database and statistical information

  • This includes preparing and submitting reports to the state or clinical reports for individual cases

  • Orient and trains new staff to the program's mission, goals and methodology

  • Ensures that staff is trained in service delivery and provides ongoing supervision

  • Maintain direct service caseload

  • Assists with direct service functions as needed

  • These include assessments, developing treatment plans, off-site intervention, behavioral skills training, clinical case management and crisis intervention

  • Collaborate with other hospital programs, community agencies and families in treatment planning for clients

  • Participate in training programs to agencies and at conferences

  • Ensure that Documentation of interventions and contacts are completed timely on the CARE-MIDD electronic medical record system in EPIC, and Maintain up-to-date and complete records of client s background information

  • Assist program director in daily Operations, implementing and monitoring client database and statistical information

  • This includes preparing and submitting reports to the state or clinical reports for individual cases

  • Participate in All Project ECHO sessions as integral interdisciplinary Hub team member
