As a Clinical Pharmacist Specialist, you'll provide clinical pharmacy services in a professional and competent manner to assure rational and appropriate drug therapy to the patients within our Oncology program
STATUS: Full Time, 40 hours
LOCATION: Geneva Destination Campus
DEPARTMENT: Pharmacy | Oncology
SCHEDULE: Monday-Friday, day shift
ATTRIBUTES
- Completion of residency OR fellowship training OR board certification through the Board of Pharmaceutical Specialties. The employee may be working towards board certification as part of a timeline mutually agreed upon by the employee and supervisor
- Licensure as a Pharmacist in New York State required
- Possess a broad scope and depth of pharmacotherapy knowledge and clinical skills. Independently responsible for complex decision-making, such as drug therapy selection
- Works in collaboration with others across the Health System providing patient care and medication consultation services
- Able to develop, recommend, and monitor therapeutic plans
- Function on Health System multidisciplinary teams in the development of protocols, order sets, and critical pathways to optimize patient care.
- Works with pharmacy management and specialty departments to implement policies and procedures to ensure compliance with all applicable regulations
- Detect, prevent, monitor, document, and report adverse drug events and medication errors
- Respond to patient care urgencies/emergencies within your area of expertise
EDUCATION:
- BS: Pharmacy (Required)
LICENSES/CERTIFICATIONS:
- LPharm - Licensed Pharmacist - New York State Education Department (NYSED)
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
