STRIDES DPP Nurse Coach-Strides Program

Biweekly Hours: 80.00
Shift: M-F 8-5

Summary:
Works closely with the STRIDES Program Manager, Nurse Master Trainer and Medical Director to deliver the STRIDES/DPP Program in both distance learning and in-person formats. Serves as educator and coach to deliver multiple weekly interactive group STRIDES/DPP ILI programming and coaching to elicit adoption and maintenance of a healthy lifestyle by diverse program participants. Onboards new program participants. Works with Nurse Master Trainer to enhance participant engagement, class attendance and outcomes to maintain program CDC-recognition status. Works with the STRIDES Team on program/curriculum quality improvement. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.
Qualifications:
Required: Bachelor of Science in Nursing (BSN) Degree from an accredited school of professional nursing. Possess a current Registered Nurse (RN) license through the State of Ohio Board of Nursing. Current certification in Basic Life Support or obtains within 3 months of hire. Certification as DPP Lifestyle Coach or obtains within 3 months of hire. Demonstrated knowledge of the principles of behavior change. Ability to guide behavior change efforts without prescribing personal actions or solutions to achieve positive lifestyle changes. Demonstrated ability to build strong relationships with individuals from different backgrounds and cultures and build community within a group. Demonstrated proficiency with Microsoft Office. Proficient in multi-tasking and problem-solving. Must be comfortable working in a fast-paced environment, requiring high mobility and physical activity. Ability to work collaboratively and flexibly and adapt to changes in the work environment are essential. Ability to manage multiple and/or simultaneous responsibilities, and to prioritize one’s work schedule. Ability to maintain confidentiality for all medical, financial, and legal information. Ability to maintain positive working relationships with other employees and medical staff. Excellent verbal, written, and interpersonal communication skills, and excellent problem-solving skills. Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds.
Preferred: Experience in group facilitation using a videoconferencing platform. Experience in motivational interviewing and 1:1 lifestyle change coaching. Knowledge of basic health, nutrition, healthy lifestyle, and fitness principles contained in the CDC-approved curriculum. Knowledge of National Diabetes Prevention Program standards. Knowledge of basic EPIC functionality.
Physical Demands: Ability to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating. Ability to carry equipment to MetroHealth sites for weigh-in sessions (e.g. professional scale, stadiometer, materials, laptop). Ability to remain still for extended periods, including sitting and standing. Ability to communicate in face-to-face, phone, email, videoconferencing, and other communications. Ability to read job related documents. Ability to use computer.
