General Purpose:

The position is responsible for ensuring that hospice patient care is coordinated and managed appropriately. The role serves as a RN in hospice services through identification of residents for hospice, education, supporting transition to hospice and coordinating hospice care in collaboration with residents, families, staff and medical personnel.  Provides the lead role in clinical orientation of all new employees, conducts supervisory visits and competency assessments for annual clinical evaluations. Leads the hospice quality and regulatory initiatives through oversight of clinical documentation and clinical care standards in accordance with CMS regulations.  Participates in the on-call rotation.

Essential Duties and Responsibilities:

  • Maintains a working knowledge of current Hospice Conditions of Participation, coverage guidelines and state regulatory requirements: admission criteria, eligibility and documentation requirements.
  • Demonstrates leadership qualities, serves as role model in setting positive, attainable expectations, and goals for the clinical teams.
  • Complies with accepted professional standards and principles.
  • Clearly communicates clinical care expectations; fosters a sense of commitment and ownership among staff.
  • Fosters positive work environment and culture that promotes unity and collaboration between skilled, AL, IL staff and hospice.
  • Supports and guides clinician to promote more effective performance and delivery of quality hospice services.
  • Serves in a lead role for oversight of the Hospice QAPI program including the annual plan, quality initiatives and monthly chart audits for accuracy and compliance. Ensures that the medical records and all clinical documentation is maintained in compliance with federal, states regulations and company Policies and Procedures.
  • Assist clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care; serves as clinical resource to team and to referral sources.
  • Ensures clinician compliance with documentation requirements, timely submission of documentation, corrections of documentation errors.
  • Prepares for and facilitates and bi-weekly IDT Hospice meetings to facilitate care coordination.
  • Provides liaison services to residents, nursing staff, medical personnel and to patients/families.
  • Assists Director of Home Health Services and VP Health Services in the implementation of strategic initiatives.
  • Promotes HH and Hospice services in community; participates in selected community education/outreach activities.
  • Participates in public relation and community activities that promote the organizations role as an effective member of the home health and hospice care delivery system. Promotes customer service orientation to all home health and hospice personnel.
  • Collaborates in the employee recruitment and interviewing for Hospice with Human Resources and the Hiring Manager.
  • Oversees Hospice employee orientation; delegates orientation activities as appropriate.
  • Coordinates supervisory visits and competency assessments with other members of the Clinical Team.
  • Participates in QAPI program
  • Participates in sponsored in-service training
  • Other duties as assigned by the Director of Home Care and Hospice or the VP of Health Services

Other Related Duties

  • Provide coverage for Independent Living Nurse as needed.
