University Hospitals Health System Principal Project Manager - Physician Directed Payment Program in Cleveland, Ohio

Description Position Summary/Essential Duties: This role will oversee maternal value based care initiatives to improve maternal and birth outcomes with a specific focus on UH Physician Directed Payment Program which is designed to preserve and promote access to medical services for Medicaid beneficiaries and underserved populations. This role will work with multiple teams and stakeholders throughout MacDonald and Rainbow and the UH system to monitor the maternal and birth outcomes evaluation plan inclusive of metrics, baselines and improvement targets. This role is focused on the achievement of project success and traceability of the project goals while managing conflict between projects. Clinical Performance Improvement & Quality: working with local and system leadership to improve clinical performance and implementing needed changes to improve care and outcomes. Collaborates with quality and safety leadership to improve value-based outcomes and improve Mac/RBC/system performance in value-based care programs. Partners with leadership to assess gaps, design and implement best practices, to redesign work flows, and to evaluate progress toward stated goals. Collaborates with Quality Ieaders and organizational leadership to implement annual Quality/Risk/Patient Safety plan and develops a work plan related to system performance improvement activities. Collaborates to develop and communicate an annual work plan for quality and performance improvement, inclusive of value-based care initiatives and innovation strategies Monitors compliance with work plan relative to quality and performance improvement. Ensures hospital and population-based processes exist for identification and management of system performance improvement priorities. Leads performance improvement activities related to organizational goals and performance opportunities. Utilizes data analytics and performance improvement tools and methodologies as part of overall project management initiative. Develops and oversee an educational program that expands staff knowledge on aforementioned performance improvement tools. Collaborate with other Principal Project Managers. Clinical Risk Management / Patient Safety: working with local and system leadership to improve safety and mitigate risks to the organization, and implementing needed changes to mitigate those risks. Partners with leadership to assess gaps, design and implement best practices, to redesign work flows, and to evaluate progress toward stated goals. Leads an effective program to identify and mitigate clinical risk and reduce patient harm. Collaborates with Quality and Safety leaders and organizational leadership to implement annual Quality/Risk/Patient Safety plan and work plan as it impacts Clinical Risk Management and Patient Safety. Ensures hospital processes exist for identification and mitigation of adverse outcomes, patient safety events, and action items. Coordinates with UH Legal Department to assure risk management and patient safety issues are addressed system-wide in a consistent, standardized manner. Leads performance improvement activities related to patient safety and reduction of adverse events and monitors, measures and assists in driving iterative improvements across MacDonald/Rainbow related maternal populations. Develops and ensures implementation of an effective Patient Safety/Zero Harm/Learning from Defects Program. Guides execution of Learning from Defects and Root Cause Analysis processes to promote learning and reduction of repeat adverse events. Collaborate with other Principal Project Managers.
