Description * Community health workers collaborate with healthcare providers in assisting patients with education and support to improve healthcare outcomes. * They provide intensive case management to patients and assist with access to care, barriers in obtaining necessary resources; both community and insurance based. * In addition, they will act as a guide in navigating transitions of care during a patients health care journey. * The location for this position is the Cleveland Medical Center, Main Campus. * This position is full time 40 hours per week, days. * The population will be Emergency Services.
