Participate in outreach to | | community partners to help coordinate services for clients in order | | to help sustain sobriety. Maintain regular contact with clients to | | monitor and assess progress. | +-----------------------------------------------------------------------+ | Position Description: | | | | - Assist staff with caseloads through regular client contact in | | community and program setting. Track and assess progress as | | determined by program guidelines | | - Help to develop and implement Individual Service Plans | | - Efficiently operate personal computer to enter, edit and/or | | retrieve data using agency electronic health record | | - Assist clients in accessing community resources, including, but | | not limited to: food, clothing, supportive housing, Medicaid, job | | readiness and/or placement, public assistance, etc. | | - Facilitate site-based daily support/peer groups | | - Visit, support, and transport clients as needed upon discharge | | from program | | - Collaborate with family members, internal and external case | | managers, and other providers while supporting clients recovery | | - Prepare documentation in compliance with contract, regulatory and | | Agency requirements | | - Other duties as assigned | +-----------------------------------------------------------------------+
Qualifications
+-----------------------------------------------------------------------+ | Required Knowledge, Skills and Abilities: | | | | - Knowledge of substance use disorders and community resources to | | assist with maintaining sobriety | | - Effective verbal and interpersonal communication skills | | - Ability to operate personal computer to enter, edit and/or | | retrieve data and to produce reports and correspondence as | | necessary | | - Beginner level problem-solving skills and ability to multi-task | | while prioritizing tasks | | - Services will be provided within various Talbert House Programs | | and within the community | +-----------------------------------------------------------------------+ | Job Requirements: | | | | - High School Diploma or High School Equivalency | | - Must be in recovery for minimum 2 years | | - Peer Support Specialist certification or ability to obtain within | | first three months of employment | | - A valid drivers license and current auto insurance may be | | required | +-----------------------------------------------------------------------+ | Essential Functions/Physical Demands: | | | | - Positional: Driving in accordance with job duties assigned. | | Infrequent standing and walking.
