Become an Essential Part of Patient Care as an Inpatient Clinical Pharmacist! Are you a skilled clinical pharmacist looking to make a direct impact on patient care in a hospital setting? We are seeking a dedicated and knowledgeable Inpatient Clinical Pharmacist to join our multidisciplinary team and play a crucial role in ensuring safe, effective, and individualized medication therapy for our patients. As an Inpatient Clinical Pharmacist, you will collaborate closely with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes. You'll provide critical expertise in drug therapy management, medication reconciliation, and patient education, ensuring that our patients receive the highest standard of care.the Clinical Pharmacist collaborates with other health care providers to ensure appropriateness of all elements of pharmaceutical care and independently operates within policies and procedures to manage the department's clinical and consultative services. The Clinical Pharmacist supervises the work of supportive team members to ensure accuracy and efficiency of distributive services. Additional responsibilities include, but are not limited to serving as a drug information resource for TriHealth providers, direct patient interaction, precepting of pharmacy residents and/or student pharmacists, and contribution to department projects or initiatives in collaboration with Pharmacy Leadership. Minimum Job Requirements: * Bachelor's Degree in Pharmacy * Active pharmacist license in the state of OH * Prescription formula and mathematics for calculating dosage and quantit * Working knowledge of all Ohio Board of Pharmacy regulations * 1-2 years experience Pharmacy Job Responsibilities: * Creates and maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm , profesional environment. Establishes effective collaborative working relationships and open and active communication with pharmacy leadership, physicians, nurses, pharmacists, and technicians. Clearly articulates ideas and thoughts through written and verbal communications. * Contributes to direct patient care through careful review of patient information, assessment and provision of appropriate pharmaceutical recommendations. Promotes the correct and appropriate use of medications by maximizing the clinical effect, using the most effective treatment, minimizing the risk of treatment-induced adverse events, and minimizing the expenditures for pharmacological treatments. Communicates findings and recommendations to other health care professionals who are responsible for the patient's care. Ensures that issues are resolved before dispensing. Provides education/counseling to patients about drug therapies, and provides concise, timely responses. * Ensures the 7 patient "rights" are respected: right patient, right dose, right route, right time, right drug with the right information and documentation. Reports medication errors and assists in assessing the risks and stratigies to manage the risk. Ensures compliance with regulatory requirements and organizational policies and procedures. Provides a consistent process of patient care to ensure the safety of the patient's medication use. Assists in protocol development, implementation, and optimization. Undertakes projects that will improve the quality of patient care and promote cost effective use of medications. * Provides drug distribution skills include verification, order entry, clarification as needed, accuracy of checking, technician supervision, and timely delivery. Utilizes clinical knowledge and experience to resolve health problems through rational use of medications. Manages the drug inventory by adjusting par levels, and avoids stock-outs and decreasing waste. Communicates with the Pharmacy Buyers to ensure supply matches demand.
