JOB REQUIREMENTS: Essential Duties and Responsibilities include the following. Other duties may be assigned. Completes face-to face visits with hospice patients as requested/required. Performs assessment of hospice patients as required for hospice re-certification. Reports hospice patient visit findings to the Health Division Manager or HHC/Hospice Supervisor, and the Hospice Medical Director, as needed. Observes and reports to Health Division Manager or HHC/Hospice Supervisor any changes in patient\'s condition and needs. Complies with all applicable rules and regulations including the most current Hospice Conditions of Participation, HIPAA, and agency policies and procedures. Maintains records and completes reports as instructed by the Health Division Manager or HHC/Hospice Supervisor. It is unlikely an employee will perform all the duties listed on a regular basis, nor is the list exhaustive in the sense it covers all the duties an employee may be required to perform. Comprehensive knowledge and skill in current nursing practice. Comprehensive knowledge of interprofessional and interfamily relationships in the implementation of nursing/medical care plans. Thorough knowledge of pain and symptom management. Thorough knowledge of Hospice philosophy.
