Summary:

Develop and lead implementation of comprehensive community-health equity initiatives to improve health disparities across the life span. Collaborate, develop and implement strategies that result in improved health care services, practices, and programs that effectively address identified needs. Engage internal stakeholders across the Rush System on comprehensive social determinants of health-related initiatives within Rush hospitals and in the community. Manage program development, budget, implementation, quality control and evaluation. Facilitate and promote cross-sector collaborations that bring together stakeholders from inside and outside the Rush health care system to address disparities in health outcomes. Lead strategic development and implementation of Community Health Worker (CHW) Hub; including management, deployment and development of CHW team across the System. Strong knowledge of and ability to support the measurement, assessment, and articulation of the impact of community health strategies. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

We are looking for someone who has prior supervisory or management experience. Ideally, someone who has managed a large team.

Required Job Qualifications:

• Master's degree in public health, public policy, social work, business administration, non-profit management or related area required.

• 7-10 years of program management experience.

• Prior supervisory or management experience.

• Must be able to travel to all Rush hospital sites including community partners across the greater Chicago region. Approximately 20-25%.

• Must be self-motivated, accountable and responsive to internal and external clients.

• Understands and has ability to inform leadership on equity issues in healthcare, social services, law, policy or other related areas.

• Demonstrated success developing and evaluating program models, selecting and successfully operationalizing innovative programs.

• Ability to build internal and external partnerships. Organizational awareness/savvy.

• Must keep up to date with all national, regional, and local trends in community health and benefit and serve as a content expert for Rush.

• Strong organizational skills and attention to detail.

• Experience working with a diverse staff and communities of color

• Strong project management skills managing complex, multi-faceted projects to meet deadlines and desired results.

• Strong planning and implementation skills, including problem solving and troubleshooting skills.

Responsibilities:

Strategic Planning and Implementation

  1. Leads and/or supports the strategic planning, implementation, evaluation, and funding of community health programs at Rush University Medical Center, Rush Oak Park and Rush Copley.

  2. Gathers and reviews data to inform program development, grants, and measurements.

  3. Collaborates with stakeholders on program development and implementation, policy, events, community engagement and other initiatives.

  4. Develops and maintains strategic partnerships with community, government and other stakeholders to address health inequities in communities; including virtual and in-person outreach, education to community members and other community-based organizations. Promote continuous quality improvement processes.

  5. Responsible for analysis, evaluation and reporting of program data for quality improvement and to inform leadership and funders.

  6. Works collaboratively with community health & benefit teams to develop and assess innovative approaches to community health through the Community Health Needs Assessment and Improvement Plan. Collaborates with team lead on program implementation, goal setting, data collection and reporting of community benefits for Rush University Medical Center, Rush Oak Park Hospital and Rush Copley in accordance with state and federal requirements.

Management and Development

  1. Responsible for managing staff and day-to-day operations of the Community Health Worker (CHW) Hub and assuring execution of CHW partner programs in clinical and community settings.

  2. Collaborating with community-based practices, research and service line leaders. Assist with development of CHW care model designs, care pathways, etc. Organize, facilitate and oversee CHW training. Manage staff of 15+ community health workers, lead community health workers, and project managers. Recruits, hires, manages, and oversees training and orientation of all program staff members, including project manager for Rush Oak Park. Develops and implements a system to manage and evaluate the skill, experience and professional development needs of all staff.

Stakeholder Engagement

  1. Builds and maintains relationships with community leaders including organizations, elected officials, public agencies and other key community stakeholders to engage in strategic community health equity work.

  2. Serves as a knowledgeable representative of Rush on collaboratives and working groups.

  3. Engages local and system leadership and other key internal stakeholders to support execution of community health equity strategies.

  4. Represents Rush on coalitions, working groups, advisory boards, committees, panels, planning groups, etc. Provides assistance to these groups on topics relevant to health care, policy and systems change strategies and health disparities.

  5. Engages internal stakeholders to align community needs and business imperatives, identify and deploy organizational resources that support community health improvement.

Budget and grants management

  1. Identifies resources to support community health initiatives.

  2. Contributes to grant proposal development, data collection, reporting and budget management.
