Chesapeake Regional Healthcare Nurse Mgr, Home Care or Hospic in Chesapeake, Virginia

The Home Care and Hospice Nurse Manager is responsible for developing and leading a multidisciplinary team in the achievement of clinical program outcomes, ensuring that daily staffing needs are met, ensuring that patients receive the care described, and ensuring that care is administered safely and effectively by the appropriate personnel.

Essential Duties and Responsibilities

These duties and responsibilities described below represent the general tasks performed on a daily basis; other tasks may be assigned.

§ Financial Management:

o Assist in overseeing the financial performance and productivity of the clinical team within the specified fiscal parameters

o Assist in preparing yearly fiscal goals and budgets based upon clinical goals and standards

o Develop a personnel schedule that demonstrates the ability to manage resources to meet volume and acuity

o Assist in developing, implementing, and managing cost reduction and revenue initiatives within the department

o Identify and explain budget variances to the Director

o Prepare monthly team specific report card and advise Director

o Monitor department payroll to assure adherence to policies by staff and eliminate unnecessary overtime

§ Quality and Service Excellence:

o Create a culture of service excellence that ensures patient, family, physician, and co-worker satisfaction

o Analyze workload to determine the number of employees needed to ensure adequate staffing, recognizing that staffing is a team effort

o Direct, plan, and schedule the work of others during shift

o Demonstrate and model consistent ability/competence in patient care areas

o Consistently establish professional standards for quality, legal, and ethical nursing practice

o Collaborate with the Quality Assurance/Quality Improvement Coordinator to develop and monitor unity quality improvement plan(s)

o Establish collaborative relationships with physicians and other Nurse Managers to achieve collaborative practice outcomes

o Recommend and implement changes in clinical practice and/or process based on clinical findings, multidisciplinary recommendations, standards of practice, and research

o Create and maintain an environment that promotes patient and staff safety

o Consistently meet and comply with Joint Commission standards and regulations as well as all relevant regulatory agencies

o Comply with and ensure staff compliance with established nursing standards

o Monitor compliance by team members of organizational policies, procedures, and objectives regarding quality assurance, safety, environmental, and infection control

o Interpret these to subordinate personnel, patients, medical staff, and the public as appropriate

o Document and submit follow-up of problem solving to the director involving valid patient complaints, medication errors, and patient incidents according to established policy

o Evaluate staff and identify areas of performance excellence and areas in need of further development

o Exhibit skilled performance in managing stress

o Demonstrate a willingness and ability to initiate and implement changes

o Act as a liaison between patients, families, and healthcare providers

o Assist in developing unit-based systems that support staff risk taking

o Support the creation of teamwork and team spirit both interdepartmentally and intra-departmentally

o Provide structure that facilitates emotional and situational support for peers and nursing staff

o Support timely and effective patient throughput and other initiatives that encourage and enhance the patient's home care or hospice experience

§ Human Resource Management:

o Act as the Chief Retention Officer and perform Human Resource functions when needed

o Assist in the recruitment, selection, and retention of qualified staff whose performance and values demonstrate the goals of the team

o Consistently monitor and evaluate staff performance in accordance with established standards

o Proactively deal with conflicts consistently, fairly, and in a timely manner

o Assist in the formulation and implementation of strategies for professional advancement and retention

o Encourage, maximize, and provide learning and career opportunities for staff

o Support an environment that promotes innovation and creativity

o Effectively communicate the vision, mission, and goals of Chesapeake Regional Medical Center and the department, including hospital and unit specific dashboards

o Foster an open work environment that is conductive to teamwork and encourages communication

o Follow administrative protocols

o Create a culture within the department that rewards cooperation, teamwork, and collective problem solving

o Manage conflicting priorities using effective time management skills

Supervisory Responsibilities

Reports to: Director, Home Care Services

Supervises: Clinical nursing staff and CNA staff

Responsibilities: Day-to-day management of clinical staff

Education and Experience

Minimum Required Education: Graduation from an accredited Registered Nursing Program; must have BSN or be actively working towards completing BSN degree

Experience: 3+ years' supervisory/administrative experience required; home care experience is preferred

Certificates, Licenses, Registrations

Applicant must be licensed to work as a Registered Nurse in the Commonwealth of Virginia, and must have current BLS and CPR certifications; certification in specialty area is preferred.
