The Cardiac Electrophysiology and Advanced Heart Failure Clinic-Fayetteville Registered Nurse Coordinator is responsible for coordination and management of care with patients and families with cardiac arrhythmias, advanced heart failure as well as fulfilling the clinical RN roles in our outpatient clinic. This is a unique opportunity to become a member of a dynamic team providing access to world-class cardiac care.

Prefer candidates with at least two years of experience in Cardiology, Care coordination, and/or quality improvement.

Summary:

Provides clinical administrative nursing care coordination in support of a patient care area. Duties include one or more of the following care coordination, discharge planning, data analysis and performance metrics, and patient/staff education.

Responsibilities:

  1. Assesses patient status of a specialized population. Rounds with an interdisciplinary health care team to determine patient care needs and coordinate ongoing acute care and long-term needs. Uses expertise and experience in specialty practice to facilitate patient care. Acts as a liaison between the patient, family and health care team to communicate patient care needs including coordination of patient/family care conferences.

  2. Collaborates with health care team to provide information and resources to facilitate patient care. Provides inservices and formal presentations to nursing, medical and ancillary staff on topics related to the specialized patient population. Assist staff in the development of patient education materials for the patient population. Perform patient teaching related to specific topics and patient needs. Documents patient teaching in the medical record in the appropriate sections

  3. Collects data related to specialized patient population and analyzes for trends. Enters data or coordinates its entry into databases and registries for benchmarking purposes. Participates in performance improvement activities to promote quality patient care. Analyzes data for trends and makes suggestions for revisions to patient care protocols for the patient population. Monitors outcomes of care for patient population and collaborates with the interdisciplinary health care team on evaluation of outcomes.

  4. Coordinates patient care for post-hospitalization care and discharge planning. Makes referrals to other care providers during acute care episode and for post-discharge care. Communicates discharge needs to interdisciplinary health care team, patient and family. Coordinates appointments with clinics, outside referral sources, home care, durable medical equipment providers and other providers to ensure continuity of care for the patient. Attends care conferences and interdisciplinary care rounds. Documents care planning and communication with health care providers in the medical record.

  5. Promotes own professional development through attending inservices and presentations. Keeps current in practice through conferences, reading current literature and research. Role models professional nursing practice to others through use of the nursing process and professional behaviors

Additional Responsibilities:

  1. Clinical:. Performs tests (EKG, Ziopatch placement) under the direction of a provider. Assists the provider as directed, during examinations if needed (i.e. six-minute walk test). Reviews the patient record to determine whether patient requires any routine health screenings. Provides patient with after-visit, follow-up, self-care, or procedural instructions and/or educational materials as directed.

  2. Administration: Coordinates scheduling of electrophysiology procedures (specifically Cape Fear Valley Health ). Coordinates with HF team in scheduling of cardiac testing to be completed on main campus Coordinates post procedure follow up appointment (discharge planning) to ensure continuity of care for the patient. Facilitates communication with ancillary and subspeciality services to ensure appropriate referrals completed and diagnostic testing completed. This includes assisting with prior authorizations, enrollment for patient medication assistance programs, and obtaining outside records. Communicates needs to interdisciplinary health care team, patient, and family. Coordinates appointments with clinics, outside referral sources, home care, durable medical equipment providers and other providers to ensure continuity of care for the patient. Coordinate scheduling, results review, and patient notification.

  3. In the Liaison and Transition role, you'll act as the crucial link between patients, families, and healthcare teams, ensuring seamless communication of patient care needs. This includes developing patient care order sets and implementing systems to facilitate efficient care delivery within the patient and family-centered care model. You'll also serve as the front-line resource for triaging patients through various channels like telephone, electronic messaging, or in-person, while simultaneously responding to patient calls, Epic messages, inquiries, refill requests, and other communications directed specifically to Drs. Byku, Baker, and Syed from patients within the Fayetteville pool. Your coordination with physicians, schedulers, and administrative staff will be essential for prompt and comprehensive responses and actions. Education. Performs patient teaching related to specific topics and patient needs. Topics include, but are not limited to medication, imaging, laboratory values, self-management resources, and future appointments/assessments. Documents patient teaching in the medical record in the appropriate sections acquire and maintain knowledge in electrophysiology, advanced heart failure, nursing practice and assumes responsibility for the professional development of self and patients.

  4. Quality Improvement. Participates in the evaluation of the Cardiac Electrophysiology and Advanced Heart Failure Clinic-Fayetteville. Reviews patient resource utilization with administrative and clinical staff to identify areas for improvement. Participates in the development and improvement of clinical pathways.

Licensure/Certification Requirements:

● Licensed to practice as a Registered Nurse in the state of North Carolina.

Professional Experience Requirements:

● Five (5) years of professional nursing experience.
