ServiceSource is proudly hiring an on-site Nurse - LPN to provide the daily, ongoing or emergency medical needs of the individuals that attend the Chantilly Community Integration Center. Our programs provide a variety of services for adults with developmental disabilities such as: community integration, skill building, active engagement, recreation, and vocational training, as well as a variety of medical and behavioral supports at all levels of intensity.

Job Summary

The Nurse – LPN provides first aid and nursing services to monitor and/or meet ongoing and emergency needs of participants. They ensure compliance with all state and federal regulatory guidelines, including Medicaid, Licensure, CARF and other funding sources, as well as company policies and procedures.

Primary Duties

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. These duties and responsibilities will be rated on the Annual Performance Review.

  • Review quality assurance to ensure accurate medication transcription, and complete all follow up recordkeeping, per the state of Virginia Board of Nursing Regulations.

  • Administer all prescribed medication(s), per the state of Virginia Board of Nursing regulations.

  • Assess health care needs and physician’s orders and develop applicable written nursing procedures and protocols as a part of the individual support plan when needed.

  • Implement first aid and nursing procedures and protocols as needed.

  • Communicate with physicians, local health centers, family members, authorized representatives, guardians, residential providers, and Support Coordinators in an effort to assure a coordinated system of health care services.

    Additional Responsibilities

  • Provide training and consultation for employees and family members as needed.

  • Serve as a member of the Interdisciplinary Team, including new referrals, as assigned.

  • Complete all required documentation, including notes for quarterlies, assessments and protocols.

  • Promote community awareness of Intellectual/Developmental Disabilities.

  • Perform other responsibilities as assigned.

    Qualifications: Education, Experience, and Certification(s )

  • Required: Active, Licensed Practical Nurse in Virginia.

  • Required: Current Health Care Provider, BLS/CPR Certified.

  • Preferred: Experience working with adults with developmental disabilities.

  • Must pass the State of Virginia Department of Behavioral Health and Developmental Services (DBHDS) fingerprinting and criminal background check.

  • Must pass the ODD (Orientation to Developmental Disabilities) assessment and competencies.

    Knowledge, Skills, and Abilities

  • Ability to read and understand technical instructions provided in written and oral form.

  • Exceptional report writing skills with strong attention to detail.

  • Strong customer-focused attitude.

  • Ability to develop and implement professionally written individualized protocols and supporting documentation.

  • Physical ability to support participants with personal care (feeding, toileting, transferring, etc.). 

  • Ability to relate well to people at all levels of an organization.

  • Proficiency in Microsoft Office skills, and the ability to become familiar with corporate specific programs and software such as ZOOM or Microsoft Teams.

  • Excellent verbal and written communication skills.  

  • Detail-oriented and able to carry out work with the highest levels of accuracy.  

  • Demonstrates good judgement and decision-making skills.

While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to stand, walk, sit, use hands to finger, handle, or feel, and reach with hands and arms. The employee will be required to climb or balance and stoop, kneel, crouch, or crawl. Position requires bending, standing, and walking the entire workday. Must have the ability to lift 10 pounds frequently and up to 30 pounds occasionally. Specific vision abilities required by this job include close vision, distance vision, depth perception, and ability to adjust focus.
