St. Joseph County ISD Early On Service Coordinator in Centreville, Michigan
PERFORMANCE RESPONSIBILITIES:
- Manage a caseload of Early On families.
- Determine a child's eligibility for Early On and prepare a professional report of findings.
- Use employer selected tools, prepare comprehensive screening of the child's family, developmental and social history and recommend appropriate educational and community services and/or referrals.
- Conduct hearing, and vision screenings per Early On program guidelines.
- Prepare required reports and provide necessary information accurately and punctually.
- Coordinate services for young children in St. Joseph County who have special needs.
- Prepare for and participate in teaming meetings.
- Assist families with referrals for emergency family support and resources available throughout the county.
- Conduct regularly scheduled homes visits for Early On.
- Support families and children before, during, and after transitions at program enrollment and exit.
- Provide child development information, parenting information and community resources to enrolled families.
- Complete and maintain appropriate paperwork for Early On to meet Michigan Department of Education and ISD requirements.
- Develop an appropriate schedule.
- Maintain infant, child and adult CPR/AED and first aid certification.
- Maintain appropriate communication log, records and reports regarding evaluations and contract with children, their families and community partners.
- Develop and nurture community relationships with partner agencies serving young children with special needs.
- Communication effectively with other staff, community partners and administrations.
- Participate in professional development activities as appropriate and/or requested by the supervisor.
- Provide personal transportation to homes and meetings throughout the county.
- Complete all other duties as assigned by the St. Joseph County ISD Administration.
