Madison County Office for the Aging NY Connects Program Coordinator in Canastota, New York
NY1517214 Madison County Office for the Aging, Inc is seeking an NY Connects Program Coordinator. This position involves the responsibility of being the first contact with the community, including clients directly who are elderly and/or disabled, caregivers and families of, that are seeking information and access to long term services for the purpose of remaining in the community rather than institutional placement. The position will be responsible for state reporting, outreach activities and a presence on various councils. This position will provide a single, coordinated system for accessing long term care services for all persons regardless of payor source, that will minimize confusion, enhance individual choice, and support informed decision making.
This employee assists the Office for the Aging in the development, coordination, implementation and monitoring of program goals, procedures, and regulations. Must be able to undergo a successful background check and reference check. Hours are Monday through Friday, 8am-4pm. Pay is $38,000 per year.
