JOB DESCRIPTION
Position: Direct Support Professional FLSA: Non-Exempt Reports To: Support Coordinator Program: MBSI, MBOE Summary: Serve as instructor, mentor, and advocate for individuals on assigned caseload. Assure all services contained in person centered plan are provided either directly or in coordination with other Support Professionals. Collect program data relative to services and outcomes. Complete program notes and reports and recommend plan goals or modification. Responsible for the safety, quality of service, satisfaction and care of the person served. Essential Job Functions: Work in the community at various job sites providing employment support and vocational
coaching, modeling, and training to people who are cognitively, behaviorally and/or physically disabled.
Serve as teacher, trainer, mentor and advocate. Mentor the person served in developing personal goals and choosing the support and
services necessary to achieve those goals. Monitor progress towards goals, collect data on all services provided, evaluate results, make
recommendations, and prepare reports. Accurately maintain and submit all required reports and documentation in a timely secure
manner. Provide or coordinate the provision of all services included in the person-centered plan. Provide or arrange transportation as assigned. Communicate and coordinate with others to maximize the effectiveness and efficiency of the
services provided. Provide for the safety of all persons served. Ensure program sites and vehicles are operated
and maintained in a clean, safe and healthy manner. Attend and actively participate in meetings as required. Attend training sessions, workshops and conferences. Adhere to all Witco standards, policies and procedures. Participate in fund raising events sponsored by Witco. Fulfills Mission, upholds Values and believe in Commanders intent, All Life Has Value. Assumes responsibility for other duties as required or assigned by Support Coordinator. Skills/Knowledge: Interpersonal Skills Sensitive and compassionate, active listener, positive, outgoing and
