Are you passionate about making a difference in the lives of high-riskpregnant women and children? We have an exciting full-time opportunity fora Social Worker II in our Care Management for High-Risk Pregnancies(CMHRP) program.
Contract Details:
Location: Burlington, NC
Contract Length: 13 weeks (extension and perm placement options)
Schedule: Mon-Fri; 8a-5p; 40hrs/wk

Key Responsibilities:
Care Management: Assess, plan, facilitate, and evaluate care for high-risk pregnant women and postpartum women.
Community Engagement: Conduct community visits, OB office visits, and home visits to meet program measures.
Child Case Management: Provide case management for at-risk children (ages 0-5) in the CMARC program, promoting optimal functioning for children at risk for disabilities.
Emergency Response: Assist with emergency shelters, public health outbreak investigations, and other duties as required by health officials.
Qualifications:
Education: Bachelor’s degree in social work or a related human services field, with relevant experience.
Skills: Strong knowledge of social work principles, medical terminology, and community resources. Excellent communication, compassion, and teamwork abilities.
Attributes: Accountability, accuracy, honesty, integrity, good judgment, and safety-mindedness.
