Spectrum Health and Human Services Senior Clinical Re-entry Care Coordinator in Buffalo, New York

SUMMARY OF POSITION FUNCTION:
Provides direct service to assigned caseloads. Services include comprehensive needs assessment, service planning, coordination and integration of services, recordkeeping, case management, outreach, and crisis intervention. Provides assistant and support to the Re-entry Task Force Coordinator.
MAJOR DUTIES AND RESPONSIBILITIES:
Knowledge and demonstration of agency core values in day-to-day activities
Provides comprehensive Care Coordination to assigned caseload with complex needs.
Assist when needed in formulating a clinical needs assessment.
Work effectively with parole and other community partners.
Work effectively with individuals enrolled to meet benchmarks regarding enrollment, Interactive Journaling enrollment, service planning and goal completion.
Completes all service planning requirements and assists with linkage to identified supports and works to reduce barriers to service access and linkage.
Provide constant outreach to enrolled individuals and their support network.
Assist with maintaining linkages to medical, financial, social, rehabilitative, addiction and psychiatric services.
Serve as a social change agent by identifying service barriers, participating in the development of solutions, and serving as a systems advocate.
Perform recordkeeping, statistical tracking, and quality assurance activities in an accurate and timely manner.
Assists with support to the team and coordinator, this may include, but is not limited to service planning, engagement strategies, identification of new or needed resources and data support.
Complete all required training for the agency and DCJS.
Other duties as assigned.
SKILLS/COMPETENCIES:
Demonstrated ability to work independently and to initiate change.
Must possess excellent skill and judgment in a range of clinical and case management activities.
Must be able to work effectively with a team and have demonstrated interpersonal skills in dealing with other providers and client support.
Must possess excellent written and verbal skills.
Proficient in the use of computer software such as MSWord, Excel, Power Point, Desktop Publishing, Outlook, etc.
EDUCATION REQUIREMENTS:
Master’s degree with a license or CASAC preferred.
Bachelor’s degree required.
EXPERIENCE:
- At least two years’ working with the re-entry population.
Must possess a valid Driver’s License with a satisfactory driving record, and possess a personal vehicle for job requirement
COMPENSATION: $51,874 - $66,024/yr and a $2,500 sign on bonus
