Spectrum Health and Human Services Community Wellness Specialist II in Buffalo, New York

SUMMARY OF POSITION FUNCTION:

Under supervision of the Team Leader of Buffalo Hope, the Community Wellness Specialist II provides emotional support and related service offerings via outreach and wellness activities. Provides screening, referral and linkage to community-based resources utilizing active supportive listening.

MAJOR DUTIES AND RESPONSIBILITIES:

  • Knowledge and demonstration of agency core values in day-to-day activities

  • Works with individuals, families, and groups to provide outreach, emotional support, individual and group sessions, public education, and referrals when needed.

  • Represents program in the community and networks with other agencies and partners to ensure needs of the community are met.

  • Provides presentations to community groups on emotional reactions, coping skills, and stress management.

  • Advocates on behalf of the client to ensure needed services are obtained

  • Provides Service Referral, Linkage, and Related Activities

  • Utilizes Motivational Interviewing techniques to increase likelihood of engagement

  • Knowledge of Solution Focused Therapy

  • Utilizes supportive, active listening skills

  • Brief educational or supportive contact

  • Support and educational groups

  • Self-help groups

  • Screening, referral, and resource linkage

  • Community support and networking

  • Public education

  • Development and distribution of educational materials

  • Frequent or occasional driving of personal vehicle for purpose of meeting with clients in the community and providing various counseling related activities.

  • Other duties as assigned.

    SKILLS/COMPETENCIES:

  • One – three years' experience in Physical Health, Behavioral Health or Human Services setting or Degree or Certification in Human Services Program is preferred

  • Must be proficient in the use of computers and computer software. Familiar with all Microsoft products

  • Training in Motivational Interviewing and Person-Centered Planning to be completed within three months of hire

    EDUCATION REQUIREMENTS:

  • High School Diploma

    EXPERIENCE:

  • One to three years in Mental Health or Human Services setting dependent on certification/education is preferred

  • Must possess a valid Driver’s License with a satisfactory driving record, and possess a personal vehicle for job requirement

    COMPENSATION: $21.00/hr
