Criminal Justice clinical Specialist
JOB SUMMARY
The Criminal Justice Clinical Specialist serves as a member of the clinical team, responsible for reviewing case specific information and consulting with Health Home Care Coordinators to assist in the provision of quality health home care coordination services for the people we support who have criminal justice involvement. In partnership with the Health Home Care Coordination team, the Specialist provides expertise, support, and information to the people we support around criminal justice issues and mitigating further criminal justice involvement. The Criminal Justice Clinical Specialist serves as the single point of contact for various judicial entities, risk management processes, and law enforcement/first responder organizations.
ESSENTIAL FUNCTIONS

Review case specific information and consult with Care Coordinators to support them in the delivery of health home care coordination services.
Assist with the development and implementation of interventions aimed at decreasing criminal justice involvement for the people supported by the health home care coordination organization.
Document activities pertaining to the people we support in the Electronic Medical Record Platform, in accordance with regulations, administrative memoranda, and Person Centered Services standards.
Complete and maintain OPWDD Risk Assessments per the appropriate Standard Operating Procedures and OPWDD guidance
Serve as the subject matter expert on Criminal Justice and people with Intellectual Developmental Disabilities by providing trainings, information materials, technical assistance as appropriate.
Serve as a member of interdisciplinary teams for people we serve with complex needs on an as needed basis
Assist with discharge planning for people integrating from a criminal justice setting to include attending discharge meetings, consulting with staff from criminal justice settings, consulting with probation/parole officers, and other judicial personnel.
Assist the health home care coordination team with advocating for people we support who have recently become incarcerated or are being detained on a short term basis
Track and provide analysis of predetermined outcomes related to recidivism, reincarceration, length of incarcerations, criminal justice involvement, etc.
Assist care coordinators with the interpretation of assessments and screening tools, recommend additional screening or assessment when appropriate
Work in partnership with Care Coordinators to assess safety concerns in an individual's environment, promote accessibility and mobility, as well as to provide instruction in individual safety.
Engage with the people we support in person in various settings as needed.
Serve as the Criminal Justice Liaison to the OPWDD Forensic Teams in Region 1 and Region 2
Complete and update the Forensic Notification Forms for weekly meetings with OPWDD Forensic Team- Region 1
Assist Health Home Care Coordinators with developing plans and strategies to support both OPWDD providers and Community Based Organizations to serve people with criminal justice involvement served by Person Centered Services.
Provide crisis intervention assistance to Health Home Care Coordinators as needed.
Operate in a trauma-informed manner.
Interact with people with developmental disabilities and their families on a regular basis.
Additional duties as necessary or assigned.

EDUCATION & EXPERIENCE
Master’s Degree in a field of psychology, social work, counseling, criminal justice required (LCSW, LMHC preferred)
At least 2 years of experience working with people with intellectual/developmental disabilities required , 5 years preferred .
At least 2 years of experience working with people with criminal justice involvement required
Experience in providing clinical /recovery/criminal justice rehabilitative services/programming preferred.
Familiarity with person-first, person-centered planning concepts, preferred.
