Hourly pay rate $22.00
The case manager assists those 60 and older with offering information and assistance and access to necessary services and supports appropriate to live independently. The Case Manager will screen, assess, develop and implement a care plan with the individual and provide referral/linkage to appropriate programs and services. Follow up and monitoring of the delivery of services will take place every two months with reassessment and termination when appropriate.
ESSENTIAL FUNCTIONS/RESPONSIBLITIES:
Provides information and referral on services to seniors, their families and service providers on the telephone, in person and through community forums. Reviews cases and determine eligibility for benefits.
Provides information and assistance to eligible seniors in service area as needed.
Provides in home assessments of seniors and develops care plans to meet needs. Implements care plans and revises as necessary.
Advocates on behalf of clients with formal and informal support systems. Provides linkages and referrals to agencies and programs providing assistance to seniors. Stays current with programs pertaining to the elderly.
Efficiently completes documentation; creates and maintains reports of client data into Erie County department of Senior Services selected software, and tracks client progress.
Acts as a community liaison to proactively create relationships with community stakeholders and key referral sources to assure program visibility.
Develops service plan as needed and monitors, minimally every two months, which may include routine telephone consultation.
Attends meetings, trainings and in-services as required by the Erie County Department of Senior Services, Obtains at least 16 hours of approved in-service training annually. .
Demonstrates the ability to successfully utilize computer software for assessments, reporting (including worker log) and correspondence.
Is responsible for maintaining client case records in an accurate and timely manner. These records will need to reflect understanding of and adherence to established EISEP standards.
Participates in regular case conferencing sessions with supervision and consults with supervisor as needed on difficult cases.
Intervenes in crisis situations, making appropriate referrals to agencies dealing with medical, environmental, mental health and utility emergencies.
Compiles and maintains Resource Center including supports in designated service area.
Supervises students, volunteers, aides or other individuals as assigned.
Maintains confidentiality; utilizes potentially sensitive information in accordance with agency and departmental policies.
Other duties as assigned.
MINIMUM QUALIFICATIONS:
Bachelor’s degree in related field. Social Work, Psychology or Human Services, from an accredited institution preferred.
Two years of social work/services experience or equivalent service coordination and experience with seniors. Working knowledge of aging process and awareness of community services and programs.
Ability to organize, plan and manage caseloads.
Must have sound judgment, flexibility, dependability and good writing skills.
NYS Drivers License that meets agency policy.
Ability to handle multiple tasks simultaneously and work with Erie County Department of Senior Services software.
SUPERVISORY RESPONSIBLILITES:
Responsible for oversight of students, volunteers, aides or other individuals as assigned.
