CENTRAL TEXAS M H M R CENTER CASE MANAGER-COC-MED CLINIC-#TBD in Brownwood, Texas

General Description:

Under the general supervision of the program manager with some latitude for independent judgment, the case manager will assess the needs, obtain and coordinate services for individuals in the Adult Mental Health program. Primary responsibilities include, but are not limited to, case management, skills training, screening, crisis intervention, developing comprehensive service plans, implementing and monitoring of services. Completes necessary paperwork and documentation for above responsibilities. Position will also provide continuity of care for children, adolescent and adult mental health population. As primary contact for staff at private, contractual and state facilities, this position is responsible for tracking hospitalizations, discharge planning, monitoring aftercare follow-up for geriatrics and monitoring staff in the implementation of aftercare plans once the individual returns to the community. Position serves as jail liaison for all seven counties. Requires frequent in-person, short term contact with individuals, families, and service providers. Must be proficient in verbal and written skills. Computer skills essential. Job requires extensive travel for community-based services provided and flexible hours and schedule.

Qualifications:

A bachelor or masters degree from an accredited college or university with a major in psychology, social services, nursing or related field required and be credentialed as a Qualified Mental Health Professional (QMHP). A minimum of 1 to 2 years experience in mental health and mental retardation field with crisis experience strongly desired. Knowledge of mental illness, mental retardation, medications, resources and commitment procedures essential in performing a wide range of crisis duties. Professionalism in dealing with community officials and resources a must with good oral and written communication skills. Computer skills essential. Must be able to work in stressful situations. Position requires travel within a seven-county area while on call. A valid Texas Drivers License with driving record to comply with CFLR is essential. If selected for this position, employee must give CFLR authorization to request copy of driving record as well as all required prescreens and background checks. Must be able to satisfactorily complete required staff development training.
