Bowery Residents Committee Intensive Case Manager Specialist - 85 Lexington Avenue in Brooklyn, New York

DUTIES/RESPONSIBILITIES:

Manage a caseload of clients who have permanent disabling conditions, such as chronic and acute mental illness or serious and disabling medical conditions that preclude them from employment. Meet weekly with each client, linking clients to medical and mental health services in the community, maximizing the receipt of appropriate entitlements, and facilitating placements into supportive housing. Complete NY/NY housing applications and assist clients with all aspects of the supported housing process. Conduct therapeutic and psycho-educational groups covering broad array of topics such as stress management, impulse control, living with chronic mental illness and decision- making processes. Related duties as assigned.

HOURS:

Full-time; 37.5 hours per week

  • Tues-Fri 10am-6:30pm; Sat 1:30pm-10pm

  • Mon 1:30pm-10pm; Tues-Fri 9am-5:30pm

QUALIFICATIONS:

Related Bachelor’s required; Master’s degree preferred. Experience working with the mentally ill/medically frail populations preferred. CPR training certification or willingness to take training class in CPR. Basic computer literacy required.

PROGRAM DESCRIPTION:

This 103-bed men's shelter in Brooklyn serves a population suffering from multiple issues including mental illness, substance abuse and serious medical problems. The program offers the full range of case management services as well as on-site psychiatric and medical care. It also incorporates a workforce development component for men who can benefit from this service. The Lexington Avenue Residence is funded by the NYC Department of Homeless Services.
