Independent Living Association Inc. Direct Support Professional in Brooklyn, New York
Provide direct care services to individuals and is responsible for communicating the developmental status and current functioning level of individuals.
Participate in case conferences, mandatory staff meetings, in-house reviews, and Interdisciplinary Treatment Team quarterly reviews.
Responsible for monitoring and recording goals and objectives accurately.
Responsible for implementing goal activity for individuals and recording the data in the individual's goal books.
Responsible for maintaining on-going communication with the individual's day programs and workshops.
Responsible for recording pertinent information in the house log on a daily basis.
Responsible for working with individuals in activities of daily living and in the development of self-help and social skills.
Responsible for maintaining a personalized and home-like atmosphere in the facility.
When the cook is off duty prepares and cooks meals as required.
Responsible for administering individual's medication.
Participates in in-service training sessions on a regular basis.
Drives agency vehicle.
Perform other tasks as required.
HS diploma/GED required.
Valid driver's license required.
1-3 years of experience working with the developmentally disabled is preferred.
Completion of mandatory 80 hours of pre-service training and 40 hours of AMAP training as provided by the New York State Office for Persons with Developmental Disabilities (OPWDD)
First Aid, CPR, AMAP, and SCIP training. Those who do not have the training will be required to pass and complete all training within the first three months of employment.
