Veterans Affairs, Veterans Health Administration Registered Nurse (Telehealth Healthcare Coordinator) in Bronx, New York

Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more Responsibilities The RN is a self-directed professional who uses the nursing process and evidence-based practice to provide patient-driven, holistic care with the collaboration of the Teamlets (Patient, Primary Care Provider, RN CM, Clinical Associate) and larger Team (family/caregiver, internal and community-based services and disciplines involved in providing care to the patient). The RN utilizes health information and telecommunication to promote therapeutic relationships and shared decision-making between the patient and interdisciplinary team across the continuum of care to support patient self-management, prevention and wellness. She/he collaborates with the PACT Program in identifying patients for the Home Telehealth (HT) program. The CM/HT Nurse's responsibilities include but are not limited to: Using the nursing process and evidence-based practice to collaborate with the Core Team and Expanded Team in developing the patient-driven holistic care plan for life. Utilizing collaborative strategies and creative approaches in the delivery and improvement of holistic care for patients and family members. The RN functions as a member of the interdisciplinary team in the planning and management of care delivery to a complex patient population, supporting the transition and medical needs of patients in the outpatient setting, decreasing the need for hospitalization when possible. Provides specific in-home telehealth technology to an assigned group of patients with specific chronic conditions and mental health illnesses. Educates the patient/family on the technology to be used, as well as assessing, planning, implementing, documenting, evaluating and coordinating care based on the needs of the patient. Monitors patient status using home telehealth technology and other Telehealth modalities e.g. VVC. Transfers patient data into CPRS and other software packages as designated. Communicates timely with the provider and PACT RN when critical information is received. Functioning in a leadership role engaging staff in active problem solving regarding clinical and professional issues, leading to an improvement in care or practice. Providing patient and family health education with a focus on self-management, prevention and wellness based on the patient's goals. Demonstrating leadership by serving as an advocate for patients, as a team player to colleagues, to enhance his/her own and the team's professional growth, development and practice. Assisting with data collection, analyzing, trending, and reporting activities as they pertain to HT. Assisting with monitoring compliance with Conditions of Participation, Joint Commission and other regulatory agencies, standards and policies. Upholding the VA ICARE core values. Citizen of the United States. (2) Graduation from School of Nursing (a) Graduate of a school of professional nursing approved by the appropriate State agency, and accredited by one of the following accrediting bodies, at the time the program was completed by the applicant: 1. The National League for Nursing Accrediting Commission (NLNAC), an accrediting arm of the National League for Nursing located at 61 Broadway, 33rd Floor, New York, New York 10006 or 2. The Commission on Collegiate Nursing Education (CCNE), an accrediting arm of the American Association of Colleges of Nursing (AACN). (c) In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration and the possession of a Certificate from the Commission on Graduates of Foreign Nursing Schools (CGFNS) will meet the requirement of graduation from an approved school of professional nursing. (3) Registration a) Condition of Employment. A registered nurse (RN) will have a current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The RN must maintain a current, full, active and unrestricted registration to continue employment with VA. (4) Physical Standards. See VA Directive and Handbook 5019. (5) English Language Proficiency. RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Preferred Experience: Educationally prepared at the baccalaureate level or higher. Master's Degree Preferred. Minimum of three years of successful clinical practice. Demonstrated ability in the areas of interpersonal relations, critical thinking, problem-solving and conflict resolution. Grade Determinations Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.
