Work Schedule: 8:00 AM to 4:30 PM

Summary:

The Wellness Coordinator acts as an advocate for community health education and prevention with Northern Light Health affiliate/strategic partners, and with community and statewide networks.

Responsibilities:

  • Coach clients, design personalized health plans, conduct assessments, provide guidance on nutrition and exercise, track client progress, and help the client overcome challenges that prevent them from reaching their wellness needs.

  • Provide support and encouragement to assist clients in achieving their best self.

  • Encourage preventive health care and screenings as appropriate for the individual.

  • Hold the client accountable to themselves by monitoring the progress towards achievement of their action plans.

  • Provides unbiased and non-judgmental support to the client becoming their co-partner in wellness.

  • For every eight clients coached a minimum of one demonstrates a reduction in risk factors.

  • Guides the development and implementation of the array of health promotion and chronic disease prevention services that the population requires given the prevalence of risks and conditions Demonstrates understanding and abides by the International Coaching Code of Ethics and demonstrates coaching competencies as outlined in departmental directives.

  • Participate in the development, coordination, and staffing community wellness initiatives.

  • Prepare & present wellness in-services/presentations as requested.

  • Identify and support individual's needs and risks based on wellness or health assessments.

  • Encourage and support individual's health and wellness needs in conjunction with their primary care provider and/or other health professionals associate with the employer network (i.e. EAP, Fitness Center, Worker's Compensation, etc. Assist individuals as necessary with monitoring and reducing health risk factors and supporting them as they make healthy lifestyle behavior change.

  • Encourage employees to participate in company wellness activities, disease management programs, care management programs, etc. and as applicable for each employee.

  • Provide wellness counseling to clients using available health risk appraisal software.

  • Follow state/federal guidelines for cholesterol measurement and maintain competency.

  • Stay current w/research related to this position. (i.e., AHA, ADA, ACS, ACSM, ICF)

  • Demonstrates understanding and abides by the International Coaching Code of Ethics and demonstrates coaching competencies as outlined in departmental directives.
