Work Schedule: Two 7:00 PM to 7:00 AM shifts & one 10:00 AM-10:00 PM shift.

Summary:

The Transfer Center Nurse consists of two distinct nursing roles divided between coordination of transfers into Northern Light Eastern Maine Medical Center and internal patient flow and bed placement. The Transfer Center Nurse will be scheduled on a rotating basis between these two job functions. At night the Transfer Center Nurse will be responsible for both. Transfer Center Nurse will be the single point of contact for facilities/providers requesting access to services or patient transfer to Northern Light Eastern Maine Medical Center. For many this is first contact they have with Northern Light Eastern Maine Medical Center. The Transfer Center Nurse can influence the experience of the customer's encounter with the hospital. He/she will obtain patient information critical to determining appropriate provider, medical necessity and level of care; coordinating phone call between the referring and receiving providers; and assisting referring facilities in selecting appropriate mode of transportation. They will prioritize calls to assure timely access to services at Northern Light Eastern Maine Medical Center. In the bed management role, the Transfer Center Nurse will be responsible for facilitating patient flow, balancing bed requests with priority of need.

Responsibilities:

  • Maintains a working knowledge of current Transfer Center/Bed Management directives

  • Utilizes the screening guidelines to connect the appropriate providers

  • Works with providers to achieve discussion or acceptance and the most appropriate bed placement for patients.

  • Obtains clinical updates on patients awaiting placement in the region

  • Manages bed placement for NL-EMMC

  • Collaborates with Patient Flow Manager and House Managers on flow prioritization

  • Facilitate training/orientation of new employees

  • Assumes responsibility for NL Health required continued education and owns professional growth

  • Performs other duties as assigned or required
