Job Description
Summary:
Plans, implements, renders and evaluates physical therapy services in the home care setting using professionally approved assessment and therapeutic skills and modalities in accordance with the scope of practice for a Licensed Physical Therapist in New York State.
Education And Credentials
Responsibilities:
Bachelor Degree in Physical Therapy required. Licensure to practice as a Physical Therapist in NYS. Valid NYS drivers license.

One (1) year experience in Physical Therapy.
