In our time of Company growth, we are seeking an HIV Nurse Care Coordinator.

Work Location: Bradenton, FL

Under the supervision of the Director of Care Coordination, will support HIV grant programs to ensure routine screening while identifying newly diagnosed individuals or those out of care for linkage to HIV treatment and support services.

The primary responsibility of the HIV Nurse Care Coordinator is to manage care for patients with HIV. The HIV Nurse Care Coordinator's role is to work in conjunction with medical providers and social support to provide case management; education, outreach, and logistical support to assist patients in successful linkage to care, retention in care, and achieving optimal health outcomes. Additionally, the individual is responsible for patient education on the processes for obtaining and adhering to medication. The HIV Nurse Care Coordinator will be the point of contact for all assigned patients, ensuring continuity of care and services through the referral and treatment process.

As part of this role, you will:

  • Responsible for day-to-day coordination of HIV patient medical care activities including the entry of data and tracking of HIV services
  • Identify patients for inclusion in the Program utilizing available metrics
  • Perform comprehensive patient assessments upon entry to service and periodically to include patient and family support system and needs, utilizing tools as required by funders
  • Develop and implement an individualized patient care plan (that is goal-driven) with a focus on disease management and patient empowerment
  • Assist patient and family with navigation of their health care system

What you need to bring to this role:

  • RN or LPN with a minimum of 1-year experience working with Hep C and HIV patients
  • RN or LPN with two years of experience performing similar tasks
  • Strong interpersonal communication skills, excellent public speaking abilities, strong written and verbal English skills
  • Experience working in a healthcare environment with interdisciplinary teams
  • Experience working with patients with multiple diagnoses of HIV, HCV, mental illness, and substance abuse as well as diabetes and heart disease
  • Must have reliable transportation with the ability to conduct fieldwork
