Description
We are offering a short-term contract employment opportunity for an Associate Patient Care Coordinator in the healthcare and social assistance industry. The Associate Patient Care Coordinator is based in Boulder, Colorado.
Responsibilities:
• Oversee the coordination of patient care services to ensure smooth operations.
• Handle patient inquiries, addressing their concerns and needs in a prompt and detail-oriented manner.
• Employ excellent customer service skills to enhance patient satisfaction.
• Accurately process patient data, from intake to discharge, ensuring all information is up-to-date and accessible.
• Collaborate with healthcare professionals to ensure optimal patient care and service delivery.
• Utilize healthcare-specific programs and systems, such as HealthCare.gov and TRICARE, to manage patient information.
• Leverage your proficiency in R Code to compile, analyze, and present patient data.
• Participate in video meetings and discussions to stay updated on patient care protocols and team objectives.
• Work with offshore team members, when necessary, to manage patient cases and share best practices.
• Uphold all regulatory standards and company policies in all patient care coordination tasks.
Requirements • Possess strong customer service skills, with a proven ability to interact professionally and empathetically with patients, their families, and healthcare providers
• Have the ability to use video conferencing tools effectively for virtual consultations and follow-ups
• Familiarity with the HealthCare.gov platform, with the capability to assist patients in understanding and navigating the system
• Knowledge of R Code, with the ability to utilize this skill in data analysis and patient record management
• Experience or understanding of offshore medical practices and protocols, and how they can be integrated into local care provision
• Proficiency in dealing with TRICARE insurance, including understanding of coverage details and claim processes
• Demonstrated experience with NIS (Nursing Information System), with the ability to efficiently use the system for patient care coordination
