Are you looking for a RN leadership role?  We are seeking to hire a nurse project manager in a unique leadership position as part of a multidisciplinary HIV team at the Boston Healthcare for the Homeless Program (BHCHP), a program that is nationally recognized for its innovative programming and its exceptional provision of healthcare services for individuals experiencing homelessness. This role presents an exciting opportunity for a nurse leader who possesses a strong passion for clinical nursing, nursing administration, and quality improvement within the context of a mission-driven community health center.

Hours: Full Time, Monday- Friday 8:30am-5:00pm

Responsibilities:

Clinical Nursing Duties: (50% of work responsibilities)

  • Provides outpatient primary care nursing including medical assessments and triage services for HIV team patients coming in for same-day or scheduled visits.

  • Conducts virtual, telephonic, or in-person nurse assessment and triage to assess the severity of the patient’s health concerns using approved protocols and resources to advise appropriately.

  • Implements the patient-centered plan of care, evaluates outcomes, and regularly communicates the plan of care, patient status, and progress toward goals with the patient and care team members.

  • Conducts intake assessments for new HIV team patients and supports patients in linking to HIV primary care.

  • Administers vaccines, injectable medications, and provides wound assessment and care as indicated.

  • Provides medication adherence assessments and counseling; provides directly observed therapy when indicated with the support of outreach team members.

  • Coordinates care with integrated HIV team behavioral health staff.

  • Along with other team members, visits patients at shelters and other outreach sites to provide nursing care and care coordination services and identify barriers to medication adherence and engagement in care.

  • Provides chronic disease management support for HIV as well as other common co-morbid medical conditions such as diabetes and hypertension.

  • Identifies any structural barriers patients face in accessing medical care and adhering to HIV medications, and works with the interdisciplinary team to address those barriers (e.g. housing, transportation, drug treatment, food, clothing, benefits, etc.).

  • Assists medical providers in coordinating follow-ups related to the provision of medication (such as buprenorphine and naltrexone) for opioid use disorder and other substance use disorders.

  • Coordinates preventative services for HIV team patients including, but not limited to, education, preventative medications, vaccinations, testing, and referrals.

  • Documents patient information accurately and in a timely manner in the EHR.

  • Promotes a compassionate and therapeutic environment that is responsive to the unique patient population and extends to members of the care team.

  • Demonstrates initiative and commitment to continuously improve services and processes that positively impact patient care and organizational goals.

  • Exhibits a positive attitude with patients and establishes professional and respectful relationships with the internal and external healthcare team members, agencies, and healthcare facilities.

    Nurse Management and Supervisory Duties (25% of work responsibilities)

  • In conjunction with the HIV Director and Associate Director, interviews, hires, supervises, and conducts annual evaluations of Red Team clinical nursing staff according to established personnel policies and procedures of BHCHP.

  • Oversees and monitors HIV nursing operations including direct patient care and nursing workflows.

  • Assists in the development of nursing-related policies and procedures specific to the HIV Team.

  • Coordinates and completes RN staff schedules including vacation and sick time.

  • Assures completion and tracking of required competencies and credentialing.

  • Assures adherence to standards of care and documentation requirements by nursing staff.

  • Attends and participates in all HIV team, Clinic, and Program Management meetings.

  • In collaboration with the Director and Associate Director, develops Red Team specific nursing. orientation and onboarding protocols.

  • In collaboration with the Director and Associate Director, provides quality assurance and quality improvement support for clinical nursing services as needed.

  • Develops and sustains an environment that supports excellence in nursing clinical practice through planning and implementing programming that contributes to positive patient outcomes.

  • Serves as a resource/mentor to staff regarding professional development, encouraging continuing education, and individual learning opportunities as appropriate.

  • Collaborates with nursing clinical educators and outside experts to ensure appropriate and timely training opportunities for clinic nursing staff.

  • Supports HIV Prevention and Screening staff with clinically related processes (ordering clinical supplies, tracking expiration dates).

  • Along with HIV Director, develops protocols for ordering, tracking, administration, and follow-up of patients receiving long-acting injectable medications for HIV and HIV prevention. (e.g. Cabenuva and Apretude); provides nurse training in the administration and documentation of these novel therapies.

    Quality Improvement: (25% of work responsibilities)

  • Works with the Director of HIV services to develop, implement, and oversee clinical quality management programs to ensure equitable HIV care and services of the highest quality for people experiencing homelessness.

  • Works with the Director of HIV Services, to assess core HIV clinical outcomes, (e.g. HIV viral suppression and retention in care rates), drilling down by key demographics (such as race/ethnicity, gender, housing status, etc.) to identify and address quality and equity gaps.

  • Assists the Director in planning and implementing QI initiatives using evidence-based models such as plan-do-study-act (PDSA), process mapping, Lean Six Sigma, among others.

  • Works with Data Analyst to identify and collect relevant program data for QI initiatives and for Ryan White CQM activities.

  • Works with the BHCHP Quality Committee to develop, implement, and track progress on the Annual HIV Quality Plan.

  • Attends QI trainings and participates in quality learning collaboratives as relevant.

    Qualifications:

    Basic Knowledge/Skills:

  • Understanding of ambulatory and community health-oriented nursing.

  • Experience caring for patients with substance use disorder, trauma informed care, and behavioral health conditions.

  • Excellent interpersonal and communication skills.

  • Demonstrated leadership abilities.

  • Demonstrates initiative.

  • Experience with electronic medical records.

  • Bilingual in English and Spanish a plus.

    Experience and Education:

  • License to practice nursing in Massachusetts required.

  • Graduate of an accredited baccalaureate nursing program preferred.

  • Minimum of five (5) years of nursing experience.

  • 2-years of experience working in HIV care.

  • Certification by ANAC (Association of Nurses in AIDS Care) as AIDS Certified Registered Nurse (ACRN) required within 1st year employment required.

  • 2- years nurse management experience.

  • Quality improvement experience and knowledge in variety of QI methods (e.g. PDSA, Lean Six Sigma, Process Mapping) a plus.

    Compensation and Benefits:

    NOW OFFERING A $4,000 SIGN ON BONUS

  • The compensation starts at $88,600 per year and increases based on years of experience.

  • BHCHP full time employees are eligible for our competitive time off policy of 4 weeks’ vacation, health, dental and vision insurance, 403B retirement savings plan and employer retirement contribution, and pre-tax MBTA pass program with 40% discount. In addition, eligible employees will receive yearly increases, additional compensation of seven thousand five hundred dollars added to your base hiring rate for demonstrated bilingual proficiency and the opportunity to work with local hospitals and community health centers.
