RN Case Manager Accurate Home Care Full Time $76,000 - $87,000 per year Position Summary Are you a Registered Nurse (RN) looking to provide 1:1 care, manage your own schedule, and grow your skills? Complete patient visits as needed under the direction of the Director of Home Health. Participates in the implementation and oversight of nursing and home health aide services. Conduct patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans, and discharge.
