As a Care Coordinator for our Children's Behavioral Health Home team, you will be empowered to plan treatments, connect children and their families to much needed resources and advocate for their continued growth and well-being.

This position also includes a $1500 sign on bonus, $750 paid upon 3 months of full -time employment, $750 paid after 6 months of employment.

    Duties: As a Care Coordinator, you will work with children and their families to address behavioral health needs. You will provide intake and complete assessment process when clients are referred and develop a plan of care for the child with input from the team that integrates behavioral and physical health needs and goals. You will oversee implementation of the plan and monitor progress. You will connect families with needed resources, address barriers, and make referrals. You will work closely with our school partners, and potentially co-locate within a school to enhance collaboration and teamwork. Care coordinators need to be highly organized with paperwork and time management skills.

    Qualifications:

    Bachelor's degree from an accredited four (4) year institution of higher learning with a specialization in social services or closely related field. Experience in social service preferred.
