St. Luke's University Health Network Manager, Inpatient Care Management Bethlehem Campus in Bethlehem, Pennsylvania

The Manager, Care Management is responsible for the daily oversight of operations in the Care Management Department including participation in planning, developing, and implementation of care management initiatives.

JOB DUTIES AND RESPONSIBILITIES:

  • Assumes responsibility/accountability for planning, organizing, and managing the functions necessary to facilitate care coordination, utilization management, and discharge planning.

  • Performs direct administrative functions for the department including but not limited to payroll, staffing/scheduling, organization of monthly staff meetings, ensuring yearly evaluations, competencies, and educational requirements are completed.

  • Provides staff with ongoing necessary information, education, support, and coaching to foster an environment of knowledge, critical thinking, and accountability.

  • Works closely with appropriate campus and network leadership and providers in the development and attainment of departmental, campus, and network goals.

  • Monitors and manages data pertinent to departmental goals including, but not limited to length of stay, readmissions, discharge efficiency, patient experience, and employee engagement and develops and implements solutions to address barriers to achieving goals.

  • Assures that care management services are provided consistent with the requirements of regulatory agencies and internal policies and procedures.

  • Maintains operational budgets, productivity, and fiscal responsibility in collaboration with finance.

  • Promotes customer satisfaction and addresses issues in accordance with hospital policy on complaints and grievances.

  • Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.

  • Other duties as assigned.

PHYSICAL AND SENSORY REQUIREMENTS:

Sitting for one to two hours a time, stand for two to three hours at a time, walk on all surfaces for up to five hours per day, and climb stairs. Fingering and handling objects frequently. Occasionally firmly grasp, twist and turn objects with hands and fingers. May be required to lift, carry, push and/or pull objects weighing up to 50 pounds. Occasionally stoops, bends, squats, kneels and reaches above shoulder level. Must have the ability to hear as it relates to normal conversations and high and low frequencies. Must have the ability to see as it relates to general and peripheral vision. Must have the ability to touch as related to telephone and computer keyboard.

EDUCATION:

Bachelor of Science degree in Nursing or MSW degree required upon hire.

Registered Nurse or Social Worker with current license to practice in the State of Pennsylvania or seeking Pennsylvania licensure through reciprocity (RN or LSW/LCSW) required.

NJ RN or LSW licensure required for Warren Campus. Bachelor of Science in Nursing required within 5 years.

Advanced degree and Case Management Certification preferred.

TRAINING AND EXPERIENCE:

Minimum 5-7 years of care management experience in a health care setting required.

Minimum 2-3 years of leadership experience required.
