PeaceHealth Nurse Practitioner - Hospice House in Bellingham, Washington

Description

PeaceHealth is seeking a Nurse Practitioner or Physician Assistant to assist with occasional per diem coverage in our Hospice House in connection with St. Joseph Medical Center in Bellingham, Washington.

Ideal Candidate

  • Board Certified Nurse Practitioner or Physician Assistant with 3-5 years post-certification experience that includes Hospice & Palliative Care

  • Interest in supporting a Nurse Case Manager, answering clinical questions, updating symptom management and care plans, as well as providing coverage at our lovely 12-bed inpatient Hospice House and acting as the primary provider, providing direct patient care

  • Willingness to take telephone call

The Practice

  • Our Palliative Care and Hospice team offers care, support, guidance, and reflection in medical, emotional, and spiritual areas, with a focus on preventing and alleviating suffering and improving a patient’s quality of life

  • Hours of Operation are Monday-Friday 8:30am-5:00pm; may include additional call

  • Primary workspace in the Administrative Building on the same campus as Hospice House; shared office with dedicated workstation provides easy access to colleagues

  • Beautiful 12-bed facility for inpatient care as well as dedicated Hospice at St. Joseph Medical Center

  • Epic EMR

Position is paid at an hourly rate of $63.04/hour - $79.88/hour, depending on direct NP/PA experience. Medical malpractice and tail coverage are provided as well as the potential for license reimbursement.

Job Summary

Within scope of practice and in alignment with both hospital staff bylaws and PHMGs physician supervision expectations, responsible for advanced level of nursing evaluation, clinical assessment, prescribing and monitoring medications and follow-up care for patient. Responsible for patient and family teaching.

Essential Functions

  • Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care.

  • Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral.

  • Gives directions to nursing and other staff.

  • Prescribes medications and treatments and ongoing medication management. Orders lab and diagnostic tests when appropriate and interprets test results.

  • Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications.

  • Maintains legible, accurate, and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations according to established standards.

  • Analyzes new knowledge gained from conferences, workshops, professional literature, or “hands-on training” and assimilates this knowledge into clinical practice.

  • Performs other duties as assigned.

  • Required: . Must fulfill educational requirements to obtain ARNP or PA license in State of Practice

  • Required : Nurse Practitioner or Physician Assistant- Washington or

  • Required : Drug Enforcement Admin and

  • Required : Drivers License with proof of auto insurance and

  • Required Upon Hire: Basic Life Support and

  • Preferred : Advanced Cardiac Life Support

Department / Location Specific Notes

Palliative Care:

  • Supporting management of pain and other symptoms, assesses the impact of functional status on quality of life. Leads the conscious practice of cultural humility with the interdisciplinary team cultural assessment of patients. Leads care of the patient with serious illness, guiding the team through ethical principles to represent the patients’ preferences.

  • Effectively identifies, evaluates, and addresses disease process and symptom management of the patients in Palliative care.

  • Visits patients and families at their place of residence/clinic/acute care setting. Assesses condition, symptoms and manages patient’s condition related to palliative care.

  • Participates in case conferences and interdisciplinary team meetings.

  • Educates patients and/or families about symptom care.

  • Hospice and Palliative Care Certification for Nursing within 2 years of hire.

  • ANCC/AANP certification required. (ANP-Adult Nurse Practitioner; ARNP-Advanced Registered Nurse Practitioner, FNP-Family Nurse Practitioner or GNP-Geriatric Nurse Practitioner).

Working Conditions

Lifting

  • Audiologist/Psychologist: Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Consistently operates computer and other medical equipment. Fine motor skills to be able to grasp and control medical equipment.

  • Midwife: Ability to move around area with occasional sitting. Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices. Lifting (non-patient) up to 20 lbs. Bending/ stooping/ squatting/ reaching kneeling frequently. Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally.

Environmental Conditions

  • Midwife: Exposure to biohazard, body fluids and airborne particles.

  • Time spent on site in medical/hospital setting.

Mental/Visual

  • Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).

  • Ability to communicate and exchange accurate information.
