Arnot Health Admission Screener - Taylor Health in Bath, New York

Job Description

MAIN FUNCTION

The Admissions Screener will provide support to the Continuum of Care department in coordinating the admissions process, clinical information and disseminating information and resources for admission referrals for Fred and Harriet Taylor Nursing Home, TCU and Acute Care Rehab. Reports directly to the System Director of Continuum of Care with direct line to Ira Davenport Site Administrator and Director of Nursing.

DUTIES AND RESPONSIBILITIES:

  1. Locates admission referrals by working with Arnot Health case managers, and other referring facilities.

  2. Completes pre-admission screening accurately and completely and conducts patient/family interviews for each assigned referral. Travels from one provider to another, or to the resident’s home to perform a pre-admission screen.

  3. Conducts pre-admission interviews of patients and families including pre-placement preparation of patient and family for those patients deemed appropriate for admissions. This occurs in the hospital setting, in the home, or wherever the patient is residing. Telephone consultations with family prior to actual interview may be necessary.

  4. Consults with the referring facility’s case managers, social workers, discharge planners, and physicians to assure appropriateness of referrals.

  5. Provides support and assistance to patients and their families prior to admission or referral elsewhere.

  6. Confirms insurance coverage for each patient. Obtains prior approval or authorization from insurance company as necessary. Consults with the finance department regarding financial issues as needed.

  7. Maintains a working knowledge of the services and resources available in all facilities, at all post acute levels of care.

  8. Schedules family meetings and health care team meetings as requested, or as needed to insure proper payment to the facility.

  9. Faxes forms, information records, etc. to facilities and community resources, within HIPAA guidelines.

  10. Completes documentation related to clinical information and other related items, as assigned.

  11. Effectively handles all phone contact with efficiency and courtesy and effectively.

  12. Makes decisions regarding appropriateness of admissions, with input from the interdisciplinary team. Distributes and ensures all necessary departments have reviewed admission documents as appropriate.

  13. Acts as patient advocate and resource, as necessary.

  14. Conducts educational meetings, open houses, and inservices for all types of referral providers to promote the nursing home and its objectives.

  15. Distributes the facility’s brochures and other informational materials to referral providers.

  16. Attends community functions when requested as a representative of the facility.

  17. Is responsible to oversee the TCU admission screening staff and fill in when needed.

  18. Attends all annual mandatory educational programs as required by position.

  19.   20. Has working knowledge of Corporate Compliance regulations and promptly reports any potential Corporate Compliance violations to the Corporate Compliance Officer

  21. Maintains continuing education through seminars, medical meetings, bulletins, professional journals, in-services, and JET classes.

    1. Is familiar with and demonstrates Continuous Quality Improvement principles and participates in the process as appropriate, focusing on meeting the customer service needs of the internal and external customers.
  22. Possesses the highest ethical standards with respect to discretion and regard for confidential information.

  23. Is responsible for attending all annual mandatory educational programs as required by position.

  24. In order to provide the highest quality care to our patients and residents, individuals may be required to work hours beyond those normally scheduled. Work schedules may be subject to change based on the needs of the department.

EDUCATION:

Current New York State licensed Registered Nurse, LMSW or Master’s Degree with relevant experience is required. Excellent verbal and written communication skills. Qualities of maturity, diplomacy, and ability to work well with others required.

Demonstrated knowledge of NYS Medicaid & Medicare Programs, MDS based Case Mix Reimbursement systems; and documentation requirements.

Demonstrates strong clinical and supervisory skills, ability to work independently and be highly motivated.

Discharge planning experience recommended.

Must have own vehicle. Must have current NYS driver’s license, car insurance and a good driving record.

EXPERIENCE:

Minimum 5 years experience.

CARDIOPULMONARY RESUSCITATION (CPR) REQUIREMENTS:

Basic Life Support (BLS) required within 90 days of hire date.

PHYSICAL DEMANDS:

Routine office jobs. Works in comfortable positions. Flow of work intermittent.

EXPOSURE CATEGORY:

Category II. Tasks that involve no exposure to blood, body fluids, or tissues. But employment may require performing unplanned Category I tasks
