Entity: Penn Primary Care and Specialty Practices

Department: Quality & Patient Safety

Location: 50% travel to PA/NJ practices (NJ practices to include Princeton, Cape May & Somers Point)

Summary:

The Quality and Patient Safety Manager participates in the planning, implementation and evaluation of the quality, patient safety and operational performance improvement program. The Manager serves as the project manager for quality and safety projects, while also serving as an advisor and local change agent for practices and other areas as assigned. The Quality and Patient Safety Manager provides teams with the most current PI methods and tools, including project management support. This work includes, though is not limited to, facilitation of PI projects and workshops, management and surveillance of data for PI opportunities, messaging trends in the data to departments and staff, education of staff in the latest PI methods, and responding to serious patient safety events. This position is accountable for facilitating improvement in the areas of clinical quality and patient safety, patient satisfaction and clinical operations.

Responsibilities:

  • Facilitate improvement in patient experience, clinical quality and operations

  • Serves as a resource for staff, managers, and leadership for quality and safety related topics.

  • Utilize and share with teams the various PI tools, methods and approaches including facilitating Apparent Cause Analysis (APA) and Root Cause Analysis (RCA) as appropriate.

  • Facilitates projects and workshops to support teams, maintaining a collaborative partnership with practices and the entity corporate team.

  • Utilizes the HRO principles when identifying issues, projects, or when interacting with team and stakeholders.

  • Serve as a change-agent to local practices in their efforts for quality/safety improvement.

  • Effectively influences and persuades key stakeholders.

  • Ensuring compliance of external and internal regulatory requirements such as Laboratory, Infection Control, OSHA, DOH, CMS, etc.

  • Effectively utilize Penn Medicine resources for content expertise.

  • Serves as an expert/resource for Point of Care testing procedures and competencies.

  • Supports policies and education related to the protection of PHI and HIPAA policies. Follows-up and manages issues related to HIPAA related events.

  • Coordinates schedule and completion of practice site visits to conduct Environment of Care (EOC) and Infection Control rounds. Utilizes a standard survey for site visits, provides feedback and recommendations for improvement. Monitors resolution of outstanding issues.

  • Understand, disseminate and communicate trends in data.

  • Explain the meaning of data, how it compares to benchmarks. Identify, implement and monitor action plans to address opportunities for improvement.

  • Promotes a culture of safety; assists with the Patient Safety Culture Survey and action plan.

  • Helps develop, communicate, implement and evaluate priority Safety and Risk Reduction Initiatives.

  • Oversees the training, follow-up and monitoring of Safety event submissions. Monitors event trends and action plans as it relates to quality patient safety. Responds to serious safety events and communicates to practices and leadership as appropriate. Utilizes the APA and RCA process as appropriate.

  • Manages and supports initiatives related to improving workplace safety in the practices.

  • Responsible for the creation, review, revision and communication of clinical/safety related policies.

  • Performs duties in accordance with Penn Medicine and entity values, policies, and procedures

  • Other duties as assigned to support the unit, department, entity, and health system organization

Credentials:

  • PPS, CPHQ, Lean Six Sigma or related HQS certification is required within 2 years of hire.

  • If applicant holds a clinical license (e.g. RN), proof of active licensure is required.

Education or Equivalent Experience:

  • Bachelor of Arts or Science (Required); Clinical or Health Care related field preferred

  • 5+ years' experience in planning, implementation, and evaluation of a performance improvement plan, preferably in a healthcare field (Required)

  • Knowledge of and experience in quality improvement modes, such as Lean/Six Sigma, the IHI Model for Improvement, or Human Factors Engineering required. Knowledge of TJC, CMS, and NJ/PA DOH standards/regulations (Required)

  • Masters degree in medicine, nursing, patient safety and quality, other healthcare related field (Preferred)

  • Clinical experience is preferred, although candidates with non-clinical degrees, such as MBA, MPH, or MS will be considered if there is commensurate work experience in healthcare performance improvement (Preferred)

  • Prefer prior experience in ambulatory/practice settings although hospital-based experience will be considered.

  • Knowledge of national standards and outcomes for quality, safety, and healthcare - HEDIS Measures and Value Based Purchasing, AHRQ, CME Core Measures & Value Based Purchasing, AHRQ PSIs experience preferred but classroom learning or Certified Professional in Healthcare Quality (CPHQ) or Certified Professional in Patient Safety (CPPS) accepted. Root cause Analysis experience is desired.
