Work in partnership with the care team of the Bipolar Disorder Center to deliver patient-centered care for individuals diagnosed with bipolar disorder and other psychiatric conditions. Provide discipline specific knowledge to interdisciplinary team around mental health, cultural competency, social determinants of health, and innovative ways to approach chronic illness in patients.Conduct diagnostic evaluations and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services.Develop and maintain working relationships with behavioral health providers in the community to create a network of aligned providers. Maintain a highly collaborative approach to a cohort of clinical team members, as well as the local, regional and national professional communities.Deliver therapeutic evidence-based and best practice interventions (via telehealth or in-person) including, but not limited to, skills training through psychoeducation and patient education strategies; individual, family and group services; and crisis services with linkages to crisis resources as needed. Participate in social work/departmental meetings, continuing education opportunities, academic and research opportunities to optimize patient best practice and interdepartmental collaboration. Maintain written documentation and generate charge capture for all therapy services rendered, including the initial assessment, follow-up visits, and discharge summary, consistent with organizational standards. Additional duties as assigned.
