Transition Coordination Specialist - MFP Program

Overview :

The Transition Coordination Specialist will Evaluate MFPparticipants' transition needs, develop/implement transition plans and supportaccess to demonstration services throughout participants enrollment period.This position is specific to Northern Maine geographical area.

Pay Rate : $35 / hour, 40 hours/week

13 paid holidays per year

Up to 10 days of accrued PTO, based on assignment duration

Please note that this pay range represents a good faith estimate of the compensation that will be offered for this position. The actual pay offered to a successful candidate will take into account a wide range of factors, including but not limited to location, qualifications, and additional preferred experience.

Knowledge, Skills and Certifications:

  • Minimum education requirement: Bachelor's Degree.

  • Knowledge of HCBS and community LTSS programs, including applicable regulations and policies.

  • Knowledge of person-centered planning and risk assessment Familiarity with MEDXX assessment, Mecare system and care plans.Knowledge of community resources.

  • Experience working with older adults and adults with physical disabilities.

  • Experience working collaboratively with external partners and vendors

    Skills :

  • Microsoft Office Suite, Meeting facilitation

    Duties and Deliverables :

  • Developing riskmitigation and transition plans with MFP participant. Supporting participants toaccess resources and purchase goods as authorized in plan of care.

  • Facilitatemultidisciplinary team m eetings.

  • Coordinate staffing and provide serviceauthorizations to providers Support participants through telephonic andin-person contacts.

  • Maintaining documentation related to services and supports.

  • Coordinate with Long-Term Care Ombudsman Program and Service CoordinationAgencies to provide access to services Option counseling.

  • Partnering withArea Agencies on Aging in providing resources and education to individualsabout community services

    Schedule:

    Routine Business Hours M-F, 40 hours a week, Full Time Telework position.
