Wage: Start at $27+/hour, DOE Position: Clinical Liaison LPN - Lifespark Complete Senior Living Hours: Monday through Friday, 8am-5pm. Full-time, 1.0 FTE Service Area: Northwest Metro (coverage area: Ramsey, Dayton, Rogers, Maple Grove, Plymouth, Medina, Delano, Montrose, Maple Lake, Monticello, Big Lake, Elk River, Nowthen & surrounding). Plus mileage reimbursement of .67 cents per mile. This is a hybrid role. You must be able to travel to 1 site/location at least once a day (this position is a combination of remote/work in-home environment and community, TCU, LTC, Assisted and Independent Living partnered sites). The Clinical Liaison is the primary central communication and clinical operations contact for the field-based Lifespark Complete Senior Living APPs and Physicians. The Clinical Liaison will support clinical staff delivering whole-person senior primary care services in a top-of-licenses model. This model is designed to support APP and physician work that can be done within the Clinical Liaison scope of practice. This model will maximize vocational satisfaction for each team member while efficiently bringing geriatric expertise to our clients. Position Functions and Responsibilities: Perform excellent customer service to clients, customers, partners and colleagues daily. Documentation of visits and coordination details in EMR. Direct communication with MD/NP/LM/Managers for urgent client needs. Assist with medication reconciliation, vital sign entry, skilled interventions as ordered. Assist members with urgent needs within the scope of practice of LPN. Participate in team meetings. Collaborate with clinical team and assist with exams/procedures as needed. Provide coverage when needed for other clinic staff. Timely response to inquiries by telephone, EMR, Microsoft TEAMS, email, or fax. Provide team coverage to other departments as needed. Completes all documentation in a timely manner and responsible for client record content. Maintains a strict standard of confidentiality to ensure our member's protected health information (PHI) is secure. Qualifications: Licensed Practical Nurse diploma or certified medical assistant. Experience using technology including Microsoft Office, Smart Phone, Calendaring, Clinical Software platforms (EMR), Wi-Fi, Printers, and Internet Browsing Previous hospital or clinic experience highly recommended Must possess: Planning skills Creativity and ability to solve problems Ability to effectively communicate at all levels of the o ganization, verbally and in writing Ability to maintain composure and stay positive in stressful situations Ability to be organized and prioritize multiple demands in a fast-paced environment Strong customer service qualities including empathy, positivity, desire to serve, resourcefulness and reliability Collaborative spirit and desire to be part of a team Willing to travel to multiple Senior Living sites across the Twin Cities metropolitan region.
