Summary

Blue Cross Blue Shield of Michigan (BCBSM) sponsored Collaborative Quality Initiatives (CQI) are statewide quality improvement programs whose goals are to improve quality, safety, and clinical outcomes for specific healthcare procedures or medical conditions across the state of Michigan (https://www.valuepartnerships.com/programs/collaborative-quality-initiatives/). Each CQI is organized and facilitated by a Coordinating Center, led by Program Directors and Co-directors, and a Program Manager. Michigan Medicine is home to the coordinating centers for 20 of the current CQIs.

The Michigan Social Health Interventions to Eliminate Disparities (MSHIELD) is a CQI focused on addressing health equity as a key component of quality improvement in health care (www.michiganshield.org). MSHIELDs mission is to empower CQIs to lead the future of quality improvement which achieves whole health for all people by integrating social care and clinical care, using data to drive health equity, and fostering a culture of anti-racism.

The Social Drivers of Health (SDOH) Manager will be responsible for helping to lead MSHIELDs community-clinical linkages work that implements screening for unmet social health needs in healthcare settings and linking patients to existing resources in their communities. The SDOH Manager facilitates the development of social needs screening-linkage interventions, including related activities of evaluating interventions, maintaining key partnerships, and offering consultation and education to the CQI portfolio, in close coordination with other team members. The position also includes key duties related to capacity building among our organizational partners and maintaining strategic partnerships in Michigan's communities.

Responsibilities*

Supporting Community-Clinical Linkages:

  • Support the coordination and implementation of evidence-based and community-driven community-clinical linkages to address social health needs.

  • Execute evaluation efforts to understand patient and organizational partner experiences, as well as the effectiveness of community-clinical linkage interventions.

  • Support the tracking and documentation of project progress and timelines.

  • Support MSHIELD program leaders to effectively scale and sustain community-clinical linkages across different CQIs, healthcare settings, regions, and clinical settings.

Capacity-Building, Consulting, Technical Assistance:

  • Provide content expertise related to SDOH and social health needs.

  • Collaborate with MSHIELD program leaders to consult with CQIs, hospitals, practices, and external partners about interventions to address social health needs, community engagement, and best practices.

  • Collaborate with MSHIELD team members to deliver educational opportunities related to SDOH and health equity to CQIs, hospitals, practices, and other partnering organizations.

  • Maintain a portfolio of educational materials, toolkits, and resources related to SDOH, health equity, and addressing social health needs (e.g., implementation toolkit, readiness assessments, screening tools).

Engagement & Partnership Maintenance:

  • Provide support for MSHIELD's Social Care Advisory Council, which informs and guides the teams work in integrating clinical and social care.

  • Collaborate with MSHIELD program leaders to maintain strong relationships with community and statewide partners to support MSHIELD's work to integrate social and clinical care.

  • Represent MSHIELD at community events, committees, and project-specific events.

Required Qualifications*

  • Demonstrated experience leading relevant projects with multiple partners or stakeholders.

  • Excellent organization, communication, and interpersonal skills.

  • Ability to work in a fast-paced environment, handle multiple projects simultaneously, set priorities, and take initiative while keeping supervisors and project leads informed.

  • Ability to be consistently detail-oriented.

  • Flexibility and ability to work independently and collaboratively with multiple teams and individuals.

  • Proficiency in basic Microsoft programs including Word, Excel, and PowerPoint.

Senior Level

  • Bachelors degree or higher in health sciences, public health, social sciences, or a related field.

  • Four or more years of progressively responsible experience in a relevant setting.

  • Two or more years of experience in a healthcare, social service, community-based organization or related setting.

Intermediate Level

  • Bachelors degree or higher in health sciences, public health, social sciences, or a related field.

  • Two or more years of responsible experience in a relevant setting, per above.

Desired Qualifications*

  • Masters degree in health sciences, public health, social sciences, or a related field.

  • Experience leading or facilitating implementation of social needs screening and linkages in healthcare settings

  • Experience working with or in community-based organizations in Michigan

  • Ability to work effectively with limited supervision.

  • Experience with health equity, anti-racism, or supporting the health of diverse communities.

  • Commitment to personal and professional growth, particularly related to health equity and anti-racism.

Work Locations

This department employs a remote work first policy. Office and meeting space within the health system is available as needed and in-person work across Michigan will be occasionally required, but remote work is supported and encouraged as appropriate. The MSHIELD team works remotely 4 days/week, with one on-site day for in-person work at Arbor Lakes (4251 Plymouth Road, Ann Arbor).

Background Screening

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings. Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.
