POSITION PORPOSE

  • The Registered Nurse Case Manager assumes overall accountability for patients’ discharge plans while assuming a leadership role in achieving desired clinical, financial and resource utilization outcomes.

  • This is achieved by coordinating care and service for defined patient populations during an acute care episode across the continuum of care.

  • This position is a Part-Time, benefited position, scheduled 40 hours per bi-weekly pay period: Orientation for this position would last for 6-8 weeks from 8 a.m. to 5 p.m. Monday through Friday.

WHAT YOU WILL DO

  • The RN Registered Nurse Case manager is responsible for conducting a focused assessment of patient goals, as well as, biophysical, psychosocial, environmental, economic/financial and discharge planning needs.

  • One would also review, on a concurrent basis, the appropriateness, intensity, and timeliness of the level of care, diagnostic testing and clinical procedures, quality and clinical risk issues and documentation completeness.

  • The RN Registered Nurse Case Manager will develop and document a discharge plan through collaboration with the patient, family and interdisciplinary team. Ensures that all activities to facilitate and coordinate the plan are being implemented and that the plan is modified as needed based on patients’ evolving needs.

  • This role will require discussion of the plan of care daily with physicians and care team members to ensure the continued need for acute care hospitalization, while exploring the most desirable/appropriate post-acute care setting to meet the patients' needs and goals.

MINIUMUM QUALIFICATIONS

  • Current Michigan Nursing License

  • Graduation from accredited nursing program: BSN is required

  • Experience: Three (3) years recent experience in direct patient care as an RN with demonstrated clinical expertise or equivalent experience and expertise as a Case Manager.

  • Certification: Case Management is desirable.

  • Individuals serving as case managers for the health system shall obtain CMSA certification within three years from their date of employment in the role or be in the process of obtaining certification as a case manager.
