The community you’ll be caring for: AdventHealth Hospice Care

The role you’ll contribute:

The Health Information Management Technician retrieves documents and assembles patient charts and records for filing and ensures accuracy and completeness of patient health information and medical records.

The value you’ll bring to the team:

· Provides maintenance and ensures accuracy of patient medical records

· Assembles patient medical records.

· Picks up patient charts from Inpatient Unit and other departments and brings to HIM area for processing. Assembles admission documents for filing. Reviews charts and notifies others within department of missing patient required admission signature forms.

· Communicates with physicians’ offices and other health care facilities to obtain necessary documentation and signatures. Communicates with Hospice of the Comforter Clinical Liaison and nurse Case Managers on obtaining most outstanding physicians required signature forms.

· Follow-up regularly on interim orders and Do Not Resuscitate (DNR) orders.

Qualifications

The expertise and experiences you’ll need to succeed:

KNOWLEDGE AND SKILLS REQUIRED:

· Knowledge of medical records rules and regulations

· Knowledge of patient database system (i.e. Homeworks).

· Knowledge and application of Scope of Practice laws and HOTC policy.

· Knowledge of filing and record retention requirements.

· Knowledge of intermediate to advanced application of Microsoft Office Suite including, Word, Excel, PowerPoint, Outlook as well as internet software.

· Knowledge of hospice services provided by Hospice of the Comforter.

· Knowledge and support of the Biblical philosophy of compassionate caring.

· Knowledge and application of Hospice of the Comforter policy and procedures.

· Knowledge of regulatory agencies governing hospice activities.

· Ability to communicate effectively both orally and in writing, and to make professional presentations.

· Ability to manage difficult or emotional customer situations.

· Ability to work with culturally diverse groups of both genders.

· Ability to be respectful and non-judgmental in all dealings with all individuals.

· Ability to read, analyze and interpret government regulations involving all aspects of hospice care, general business periodicals, professional journals or technical procedures

· Ability to maintain strict confidentiality of organization/employee/patient/family/caregiver information.

· Ability to write effective correspondence including letters, reports and other documentation.

· Ability to operate standard office machines such as fax, copier, calculator and mail machines.

· Ability to provide superior customer service to all contacts.

EDUCATION AND EXPERIENCE REQUIRED:

· High School diploma is required

· A minimum of 1 year of experience in health information management or medical records is required.

· An equivalent combination of education and experience may be substituted at the discretion of the Administrator.

EDUCATION AND EXPERIENCE PREFERRED:

· Associates degree or certificate from college or technical school preferred.

LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:

· Medical Assistant certification is preferred
