The Office of Mental Health is looking for a Full Time Caseworker 2.  This casework position is responsible for providing Blended Case Management Services to adults with a severe and persistent mental health diagnosis.

DESCRIPTION OF WORK The blended case manager is responsible to advocate and assist individuals in gaining access to needed medical, social, educational and other services.  The outcome of these services will focus on integration in the community, vocational/educational participation, and adequate social supports and reduce hospitalization.

Job responsibilities will include but are not limited to: Develop an individualized service plan based on strengths and needs with the individual/family.  The service plan is to be goal oriented, time specific, reviewed and updated at least every one-hundred eighty (180) days per regulations.  This service plan will be a dynamic tool used to focus upon current service needs.  It will be updated more frequently than every 180 days, if the clients' needs change.  Service plans are to be signed by the individual and the individual is to be given a copy. Assist the individual in locating and obtaining services specified in the treatment and/or service plan.  This may include home and community visits and other efforts as needed. Monitor service delivery, which includes reviewing the individual's receipt of, and participation in services.  Contact the individual/family on a regular basis to determine the progress, satisfaction with service or provider and needed revisions to the service/treatment plan.  Contact with the individual's therapist on a regular basis to determine if the person is progressing in issues identified in the service/treatment plan and if specific services continue to be needed and appropriate.  Regular contacts are made with other public agencies serving the individual. Review clinical assessment information and discussion with individual regarding unmet needs and plans for the future. Active efforts to assist the individual in gaining access to needed services and entitlement, i.e.  - SSI, SSDI.  This may include obtaining assistance from the County MH/ID Administrator through the mental health chain of command in resolving issues, providing information to help plan modifications to existing services or implement new services. Assist the individual in establishing an informal support network with family, friends, peers, community activities; i.e.  - senior centers, YM/YWCA, clubs, sports, etc.

Assist individual in identifying, accessing and learning to use community resources appropriately to meet their needs.  May include attending Initial Psychiatric Evaluation, applying for SSI, medical assistance, accessing transportation, use of library, shopping, etc.  This is done by making referrals to appropriate service providers, creating resources if they do not already exist, or providing assistance directly to the individual if no other resources are available to provide instruction. Submit documentation forms, service plans, Individual Registration for updates, Closures, Encounter Forms and Medical Assistance verification as required by BCM regulations an MH/ID policy. Attend and participate in team meetings, interagency meetings, family meetings, etc., to coordinate service provision to provide for the individual's needs.  This would include psychiatric outpatient/inpatient, psychological, neurological, partial hospitalization and transportation, recreational, social, etc.  The BCM will provide transportation to access these services only when no other transportation is available. Assist individual/family with crisis resolution and development of a crisis plan. If hospitalization is requested, the BCM must see the individual and seek alternatives and/or other supports if indicated prior to instituting 302 or voluntary hospitalization and utilize the crisis plan whenever possible. Following hospitalizati n, the BCM will review and update the crisis plan with the individual. Available for on-call duty evenings, weekends and holidays as scheduled on a rotating basis.  BCM's may be required to do at least 5 on-calls per month. Participate in supervisory conferences, BCM staff training, BCM team meetings as scheduled.  Maintain professional growth and development by attending seminars, conferences and workshops. Maintain confidentiality and professionalism at all times while performing the duties and responsibilities of this position.Work Schedule and Additional Information: Full-time employment Work hours are 8:30 am to 4:30 pm, Monday-Friday, with a 30-minute lunch. REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY QUALIFICATIONS Minimum Experience and Training Requirements:

Six months of experience as a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or A bachelor's degree with a social welfare major; or A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences. Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits. Other Requirements: PA residency requirement is currently waived for this title. Hiring preference for this vacancy will be given to candidates who live within Berks, Carbon, Monroe, Northampton, Schuylkill and Lehigh counties.  If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered. You must pass a background investigation. This position falls under the provisions of the Child Protective Services Law.
