Position Summary : The Registered Nurse Team Leader will engage in activities that support the planning, coordination and implementation of patient care under the direction of the Patient Care Supervisor. These activities include: clinical field mentoring, orientation, supervision, scheduling, coordination of clinical nursing staff in designated patient care areas. The Registered Nurse Team Leader will provide field supervision of Home Health Aides and will provide direct patient care management when necessary as directed by the Patient Care Supervisor.
Education Requirements :
Graduate of School of Nursing.
Currently licensed as a Registered Professional Nurse in New York State
Valid New York State driver’s license and access to a car
Experience Requirements :
3-5 years clinical experience in Labor and Delivery

Job Specific Competencies
Principal Responsibilities:
Provides coordination and supervision of nursing staff in designated patient care areas, under the direction of the Patient Care Supervisor. Collaborates with the Patient Care Supervisor regarding new patient assignments, triages nursing staff visits, schedules Licensed Practical Nurse daily assignments. Mentors identified staff during field visits. Provides clinical resource support for nursing staff in technical areas ( such as IV trainer, computer support). Provides medical record review and feedback in sync with field visits. Provides site preceptor activities for all levels of care.
2 . Provides Home Health Aide supervision, monthly education and orientation as needed, in collaboration with the Home Health Aide Scheduler, under the direction of the Patient Care Supervisor.
Collaborates with the Home Health Aide Scheduler regarding assignments. Plans/implements monthly support groups/inservices for Home Health Aides. Provides quarterly and as needed field evaluations. Completes annual performance evaluations under the direction of the Patient Care Supervisor. Provides clinical resource support to the Home Health Aides. Provides periodic inservices for contracted Nursing Agencies as needed.
Supports the activities of identifying Continuous Care cases, care planning, scheduling, monitoring of Continuous Care Level of Care cases, in collaboration with the Interdisciplinary Team and Patient Care Supervisor. Assist the Interdisciplinary Team with identifying patients who qualify for Continuous Care services, through participation in the Interdisciplinary Team care planning conferences. Provide direct assistance to the RN Casemanager when setting up required documentation for new Continuous Care cases. Schedule Continuous Care nurses, monitor on going needs and facilitate communication between On Call staff and Continuous Care staff, in collaboration with the site Home Health Aide Scheduler under the direction of the Patient Care Supervisor. Provide field supervision to Continuous Care nurses, providing initial field orientation of Continuous Care nurse to cases as needed. Monitor each shift documentation of Continuous Care nurses, cross referencing with electronic health record and billing to ensure compliance with regulations. Ensure timely, accurate billing is submitted to Finance, under the direction of the Patient Care Supervisor.
Patient Care Management Provide delivery of comprehensive, holistic nursing care as needed to support patient care needs under the direction of the patient care Supervisor. Participates in the IDT care planning activities Assigns tasks or delegates care based upon the needs of patients/ families and the knowledge and skills of the care providers. Utilizes nursing process to assess, initiate changes, evaluate and coordinate all care services to support patient care needs under the direction of the Patient Care Supervisor. Collaborates with community agencies providing patient/family care services to ensure coordination of all care services. Provides direct nursing visits and other duties as requested by Patient Care Supervisor, as needed.
- Evaluation of Nursing Practice
Field visits with nursing staff as necessary, evaluates, documents patient and family responses to interventions. Communicates information to the Interdisciplinary Team. Participates in the collection of data related to the Quality Indicators and participates in the site analysis of this data. Uses the results of Quality Improvement activities in the preceptoring of the Interdisciplinary Team, to support changes in practice. Participate in ongoing Quality Improvement efforts such as MISYS User Group, Continuous Care Liaison Group, Protocol Review Committees, and other activities as needed. Collaboratively identifies with Patient Care Supervisor practice improvement areas and supports implementation of measures to address improvements.
This position will cover both St. Peter's Hospital and Burdett Birthing Center
Pay ranges from $32.10 - $52.25 and is based on education and experience.
