What you will do:
Provides case management for assigned patient populations. Utilizes clinical expertise, communication and problem-solving skills to achieve optimal clinical and resource outcomes. Promotes cost-effective care by minimizing fragmentation, maximizing coordination, and facilitating patient/family movement through the health care organization. Performs patient needs assessments upon admission and at regular intervals, facilitating referrals and providing linkages to health, wellness, and post-acute care resources across the health care continuum. Promotes interdisciplinary collaboration and teamwork to progress the plan of care and discharge plan. Promotes appropriate length of stay, resource management, and care transitions to the next level of care. Must comply with all federal and state regulations surrounding the discharge process.
Collaborates with Utilization Review Nurse.
Facilitates an interdisciplinary approach to patient care.
Performs face-to-face assessments of patients/families when appropriate to identify individualized needs and challenges to a safe discharge in collaboration with Social Work
Attend and actively participate in interdisciplinary rounds to ensure that all barriers to discharge are identified and an escalation process is facilitated to mitigate delays in patient progression.
Partner with our continuum of care and post-acute network to progress care, reduce length of stay, and improve patient outcomes with the vast array of services we as a ministry provide.
Provide hand-off communication pertaining to patient’s needs to receiving peers during transition of patient off unit or to another facility.
Understand their role as it pertains to achieving key performance indicators in regards to patient progression.
What you will need:
Minimum requirements:
Current and valid NYS RN license
Bachelor of Science in nursing, or willingness to purse defined plan for matriculation in program demonstrated by ongoing course work each semester
2 years of experience in acute setting
Preferred requirements:
1+ year of experience in case management or utilization management
Case Management certification (CCM) or Certified Professional in Healthcare Management (CPHM)
Discharge planning and community resource knowledge
Maintain the highest level of customer service while completing day to day duties and responsibilities
Critical thinking skills to problem solve
Excellent time management and organizational skills
High level of efficiency to communicate in a fast pace work environment across people and teams of different backgrounds and demographics
Engage within a team setting to help promote a strong culture
Passion to learn, grow and develop in the nursing field
Pay Range: $33.10 - $44.95
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
