1.Maintain an unrestricted license(s) to practice medicine pursuant to the laws of the State of Ohio, participation in good standing in the Medicare and Medicaid programs, and all other licenses, permits and regulatory approvals required to perform the services required of him/her hereunder; 2.Maintain Board certification In Anesthesiology and Pediatric Anesthesia Board certified or Board eligible, or exception approved by Chairman. 3.Provide services to all patients regardless of ability to pay or source or type of medical coverage in accordance with Children's policies and procedures; 6. Comply with all applicable accreditation and regulatory requirements and abide by all policies and procedures of Children's; 7. Perform, with Provider's agreement, such other duties and meet such other qualifications as are reasonably specified by Children's from time to time. 8. Maintain medical staff membership and requisite clinical privileges in good standing at all times during the term of this Agreement at Children's in accordance with the Children's Medical Staff Bylaws and policies. 9. Cooperate in the timely completion of documents and forms as are required by government agencies and other entities external to Children's for the documentation of duties performed and compensation provided under the terms of this Agreement; 10. Provide direct supervision, education and evaluation of all residents, medical students, mid-level providers and allied health care professionals during scheduled time; 11. Provide all reasonable medical services and support when called upon in an emergency situation; 12.
