Olympic Area Agency on Aging Family Caregiver Support Program Coordinator MAC TSOA Specialist in Aberdeen, Washington

Family Caregiver Support Coordinator/MAC-TSOA Specialist wanted for Olympic Area Agency on Aging (O3A), based in Aberdeen WA. Provides individual counseling, support and training to unpaid caregivers through home visits, phone contacts, community education, work groups, etc; provides information and assistance/referral services; will assess, authorize, monitor, and prioritize respite care services. Conducts community outreach and education.  Specializes in eligibility determination, service authorization, and case management for consumers in the Medicaid Alternative Care (MAC) and Tailored Supports for Older Adults (TSOA) programs. May provide services to Family Caregiver Support Program (FCSP), Grandparent Raising Grandchildren/Kinship Caregivers, including, but not limited to, the Kinship Care Support Program.  Collaborates, as part of a multi-agency service delivery team, to provide a wide spectrum of necessary services to unpaid caregivers within a defined 2-county area. Maintains a caseload with parity to the Title 19 caseload.  Some travel required in Gray's Harbor and Pacific Counties.* * BA in Behavioral or Human Services and two years of paid social service experience; OR  BA and four years paid social service experience. Interviewing and counseling skills with an ability to work in a team-based environment.  Knowledge of social service or human services issues, especially dealing with older adults and family caregivers and information about community resources.
