State of Massachusetts Assistant Director of Moving Forward Program in 600 Washington Street, Massachusetts
MassAbility is searching for a driven, collaborative, and program and process-oriented Assistant Director of the Moving Forward Program. The Moving Forward Program provides a wide range of support to individuals transitioning from facilities to the community under Acquired Brain Injury Non-Residential Habilitation (ABI-N) and Moving Forward Plan Community Living (MFP-CL) waiver participants and MFP Demonstration participants. As Assistant Director of the Moving Forward Program, you will focus on maintaining internal processes and procedures and developing performance standards and case management procedures that fully incorporate the principles of Person-Centered Planning and Continuous Quality Management. You will also ensure standardized operations, procedures, documentation, and clinical support statewide for case management activities. Partner with MassAbility program and internal staff, service providers, consumers, and external stakeholders to identify and address performance issues, service gaps, health and safety concerns, and ongoing program development to ensure the entire team operates smoothly and effectively. You will also work with other partners in the MassHealth-funded waiver programs, including the Department of Developmental Services, the University of Massachusetts Commonwealth Medicine Division Waiver team, and MassHealth, to ensure that waiver services operate seamlessly and benefit participants. Finally, the Assistant Director of the Moving Forward Program will be a leader in promoting MassAbility’ s learning and participatory performance culture with an emphasis on high-performance teams, individual performance/contribution/ impact, and engagement in activities to promote and support a program/division/agency (e.g., committee membership, Roadmap workgroup membership, presenting at Town Halls, achieving learning and performance goals). As a team member, you’ll join an agency that values a learning and participatory performance culture with an emphasis on high-performance teams, individual contribution/impact, and engagement in activities to promote and support programs, divisions, and the overall agency vision and mission. /_Duties & Responsibilities (the following represent a general summary and are not all-inclusive)_/: Ensures the Moving Forward Program operates smoothly and efficiently and provides effective and professional services to Waiver participants. Work and supervise a team of three regional team managers to ensure that services are provided consistently across teams. Collaborate with the Program Director to develop policies and procedures and provide consistent guidance to staff. Act as the primary liaison with the UMass Medical School, Commonwealth Medicine, and Waiver team to ensure that waiver transfers and participant redetermination activities are managed effectively with maximum input from this team. Ensure that outreach is provided effectively and efficiently. This includes ensuring that the Peer-to-Peer program remains active and fully utilized. Ensure that most pending ABI-N and MFP-CL participants transition to the community within 365 days of determining waiver eligibility. Collaborate with the Regional Team Managers and the EOHHS Disability Housing Manager to ensure that information about housing opportunities is widely shared, that the case managers understand the different types of housing subsidies and opportunities, and that they can effectively convey this information to their pending participants. Partner with the Learning and Development team to ensure that training needs within the team are met and that staff receive solid and effective training on working with individuals with a wide range of disability issues and conditions, particularly brain injury training support. Work with other MassAbility programs to ensure that a) MassAbility consumers who require skilled nursing facility or rehabilitation hospital care are allowed to learn about the MassHealth Waiver programs and b) that ABI-N and MFP-CL participants who would benefit from other MassAbility services can access these. /_Preferred Qualifications_/: Minimum 5 years full-time experience in human services program management or long-term community service/support program, including supervision of case management staff. Knowledge of Medicaid community long-term services and support programs, state-funded disability programs and services, and direct experience implementing waiver services, with specific expertise working under HCBS waiver requirements. Experience working within or with state government. Ability to collaborate with and facilitate communication with staff. Previous experience working with collective bargaining settings. Experience designing, developing, and implementing quality assurance and risk management systems and tools, including incident reporting, case record reviews, satisfaction surveys, complaint resolution, and administrative reviews when serious health and safety concerns present to mitigate risk. Experience in case management, care coordination, or program coordination with elders and people with a range of disabilities in community settings, especially consumers with ABI. Experience supporting individuals transitioning from facilities to independent community settings by implementing person-centered, independent living, and community living service plans to ensure individual support plans representing identified needs, interests, and preferences. Demonstrated ability to effectively communicate program information and philosophy to others and support others in learning this skill. Excellent written and oral communication skills and ability to write concisely, express thoughts clearly, and develop ideas in a logical sequence.
