Queen's Health System Diabetes Educator - Inpatient & Outpatient (Call In, Varied Shifts) in Honolulu, Hawaii

RESPONSIBILITIES

I. JOB SUMMARY/RESPONSIBILITIES:

• Responsible for the development, implementation and evaluation of diabetes educational programs for patients of The Queen’s Health System (QHS).

• Provides diabetes educational services and community resources for patients, their families and/or caregivers.

• Collaborates with other health care providers to coordinate the care of the patient across the continuum.

II. TYPICAL PHYSICAL DEMANDS:

• Finger dexterity, seeing, hearing, and speaking.

• Frequent: walking, standing, sitting.

• Occasional: static gripping of an object for prolonged periods, stooping/bending, climbing stairs, walking on uneven ground, lifting and carrying usual weight of 1 pound up to 5 pounds, reaching above, at and below shoulder level, pushing and pulling up to 30 pounds.

III. TYPICAL WORKING CONDITIONS:

• Not substantially subjected to adverse environmental conditions.

• Exposed to both inside and outside environmental conditions.

• Exposed to moving mechanical parts, chemicals, communicable diseases, and body fluids.

IV. MINIMUM QUALIFICATIONS:

A. EDUCATION/CERTIFICATION AND LICENSURE:

• Must meet one (1) of the following:

o Bachelor’s degree in Nursing and current Hawaii State license as a Registered Nurse.

o Doctor of Pharmacy degree and current Hawaii State license as a Pharmacist.

• Evidence of ongoing continuing education related to diabetes care and education.

• Current certification as a Certified Diabetes Care and Education Specialist (CDCES) by the Certification Board for Diabetes Care and Education (CBDCE). If not certified upon hire, must obtain certification within six (6) months of entrance into the position.

• Current BLS certification.

B. EXPERIENCE:

• Two (2) years experience as a Registered Nurse or Pharmacist.

• Six (6) months experience in diabetes care or education.

• Prior patient education experience preferred.

• Excellent leadership and communication abilities.
